Well-site BMP's appeared to be functioing properly and no accelerated erosion & sedimentation in relation to the well-pad. Noted erosion on the access road specfically to the site, not the shared portion with 005-29757 Kenneth Atkinson 2-20. It was determined that a well permit renewal was not submitted for this permit and according to 78.65(2) if a well-site is constructed and the well is ot drilled, the well site shall be restored within 30-days after the expiration of the well permit (which would be 06/28/09). At this time the Department is requesting documentation be submitted no later than June 23, 2009, showing when work will begin and/orbe completed for well-site and access road restoration. |
