1843348 Primary Facility 2009-11-06 Routine/Complete Inspection No Violations Noted Drilling was commenced on or about 11/03/09. Met onsite with operator representative David Smith to review the E&S plan, specifically for the access road. Noted numerous concerns with the steepness of the access road and the proximity of the road to a nearby the stream and the potential for accelerated erosion & sedimentation. Mr. Smith stated that the well will is to be fraced within the next week, and that as soon as that process was completed the site would be restored and a final layer of durable clean stone would be added to the access road. This quick turnaround and the current conditions alleviated the present concerns. WQS Higgins recommended that the operator insure that all erosion control BMP's on the access road be kept in functioning condition should the weather change. The broad based dips should be kept graded and the roadside ditches and subsequent rock outlets be maintained, and any disturbed areas that will not be affected by well stimulation activities should be mulched as soon as possible.
