1917976 | Primary Facility | 2010-10-05 | Routine/Complete Inspection | No Violations Noted | Site construction was underway, drilling was not ongoing at the time of the inspection. Noted that the silt fencing had not been installed properly and that due to access road construction the road should be rocked with clean durable rock and maintained as such during well development. Contacted operator representative Walt Emquist regarding these matters. Mr. Emquist stated that the previous day the access road had been discussed and it was decided to rock the entire road up to the site with R3 rock. In addition he stated that he will have the silt fencing issue addressed as soon as possible. |
1923331 | Primary Facility | 2010-10-28 | Routine/Complete Inspection | No Violations Noted | Site construction had been completed, drilling was not ongoing at the time of the inspection. Both drill and frac pits located onsite, however they had not been lined. All access road and site erosion controls had been implemented properly and no accelerated erosion & sedimentation was observed in relation to the site or access road. |

1944040 | Primary Facility | 2011-01-20 | Routine/Complete Inspection | No Violations Noted | Drilling had been undertaken but was not ongoing at the time of the inspection. Drill pit contained drill cuttings and what appeared to be a frozen liquid fraction. The pit had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or site. |
1955889 | Primary Facility | 2011-03-09 | Routine/Complete Inspection | No Violations Noted | Drilling appeared to have been completed. Drill pit located onsite and it contained cuttings and fluids. The pit had more than the required two feet of lined freeboard. There was no observed accelerated erosion & sedimentation in relation to the access road or site. Noted that the disturbed areas which would not be utilized during well development had been mulched as an additional temporary erosion control BMP and that the entire access road had been rocked with clean durable rock. |
1968522 | Primary Facility | 2011-04-26 | Routine/Complete Inspection | No Violations Noted | Drilling appeared to have been completed. Drill pit located onsite and it contained cuttings and fluids. The pit had more than the required two feet of lined freeboard. There was no observed accelerated erosion & sedimentation in relation to the access road or site. Noted that the disturbed areas which would not be utilized during well development had been mulched as an additional temporary erosion control BMP. Operator was transferring fluids from the drill pit and rainwater collected in one of the unlined frac pits to the lined frac pit. The lined frac pit had more than the required two feet of lined freeboard. |

1975742 | Primary Facility | 2011-05-24 | Routine/Complete Inspection | No Violations Noted | Drilling appeared to have been completed. Drill pit had been regraded. Well appears to have been stimulated. Frac impoundments located onsite contained fluids and they had more than the required two feet of lined freeboard. Observed no accelerated erosion or sedimentation in relation to the access road or well-site. |
2039456 | Primary Facility | 2012-02-02 | Routine/Complete Inspection | No Violations Noted | Drilling had been completed, well appeared to be equipped to production. Brine tanks located onsite and they appeared to be in sound condition. Site had been regraded, seeded. mulched and a good portion of the site had vegetation. It was noted that approximately 1/3 of what had been the well site had been disturbed for a home and sand mound construction. This home now shares the access road with the gas well. |
2235753 | Primary Facility | 2014-01-13 | Routine/Complete Inspection | No Violations Noted | Well appeared to be equipped for production, brine tanks located onsite and they appeared to be in sound condition. Site had been restored and the vegetative cover met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation in relation to the site or access road. |
2540124 | Primary Facility | 2016-11-30 | Routine/Complete Inspection | No Violations Noted | Well appeared to be equipped for production. The Department attempted to reach two operator representatives by phone to notify the operator of the below-noted observations. At 1201, operator representative Gretchen called the Department. Gretchen stated that personnel would be directed to the location to investigate and will follow-up with the Department. The Department is requesting the following information at this time: The last time the brine tank was inspected and the tank readings from the last time the tank was emptied through today. The operator appears to be in compliance with policy at this time. Violations may be forthcoming pending operator response and investigation. |
2715858 | Primary Facility | 2018-03-27 | Administrative/File Review | In Compliance with Policy | Admin inspection in response to inspection record 2707693.
Operator representative Brandon Mayhugh contacted the Department by phone to follow-up on the Department's observations observed under inspection record 2707693. Per Mr. Mayhugh: The tanks were repaired. It is not believed that the seeps were a leak, but perhaps moisture trapped under the paint of the tanks. There was no loss of fluid per tank readings and the fluid levels were previously below these observed seeps. The operator will continue to monitor the tanks as necessary in order to ensure these seeps do not reappear. |
