Drill pit located onsite and it had more than the required two feet of lined freeboard. Noted that there was no E&S plan in the tube with the site ID sign. In addition noted a spring had emerged alongside the road and was flowing down to the backside of the site. Finally there was a portion of silt fencing that had been compromised which needed repaired. Contacted operator representative Doug Miller regarding these issues, and he stated that he will have the plan replaced, the silt fence addressed and will have the spring evaluated for erosion control measures if necessary or monitoring until site reclamation activities are undertaken.

1952061 Primary Facility 2011-02-23 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection. Drill pit located onsite and it had more than the required two feet of lined freeboard. Noted that the contents of the pit appeared to be frozen with snow on top. Observed no erosion or sedimentation issues in relation to the access road or well-site. Operator should evaluate access road and site erosion controls as the recent snow melts to insure their continued functionality.
1995074 Primary Facility 2011-08-11 Routine/Complete Inspection Violation(s) Noted Drilling was completed on 01/04/11. Well was not equipped for production and the site had not been restored. Drill pit still located onsite and it contained fluids and drill cuttings. Drill pit had the required two feet of lined freeboard, however a portion of the liner was noted as to have fallen down into the pit. The silt fencing had been compromised due to the site being left unrestored. Specifically the sediment had overtopped and pushed over the silt fencing resulting in sediment being transported offsite with runoff and a portion of it had been deposited in the woods below the site. The operator is reminded that the 9-month restoration timeframe expires on 10/04/11.
2019745 Primary Facility 2011-11-16 Compliance Evaluation No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and appeared to be in sound condition. Site had been regraded, seeded, mulched and vegetation had begun to take hold. The operator should continue to monitor the site to insure 70% uniform coverage is achieved and erosion issues do not develop with the site or access road.
2143271 Primary Facility 2013-02-27 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
