2139997 Primary Facility 2013-02-14 Routine/Complete Inspection No Violations Noted Earth moving equipment was present on site during this inspection. E&S controls were installed in accordance with the Site E&S plan except the silt socks along the southern edge of the site were displaced apparently to allow for vehicle parking. The Department recommends the displaced silt socks be restored to the positions designated in the E&S plan. The Department recommends covering the entire length of the access road with rock to minimize off-site transport of sediment onto the adjacent township roadways. E&S controls should be closely monitored by the operator to ensure effectiveness of controls and prevent off-site sediment transport.
2148563 Primary Facility 2013-03-04 Routine/Complete Inspection No Violations Noted Well was being drilled at time of inspection. Flow back pit had adequate freeboard. No improper handling of drilling fluids or residual waste was noted. E&S controls were installed according to the E&S plan. The Department suggests that the Operator continue to maintain the rock surface of the access road to prevent off-site transfer of sediment to adjacent roadways.

2159695 Primary Facility 2013-04-18 Routine/Complete Inspection No Violations Noted Drilling was not ongoing at the time of the inspection. Site and access road controls had been implemented properly and there was no observed evidence of accelerated erosion or sedimentation in relation to the site or access road. Drill pit located onsite and it contained cuttings and fluids. Pit had more than the required two feet of lined freeboard. Noted in the E&S plan the soils information indicates that cuttings encapsulation could not be performed and meet the Chapter 78.62 requirements. At this time the Department is requesting tha the operator forward documentation outling how the drill cuttings are to be disposed of, and if they are to be encapsulated what evidence was found to refute the soil conditions outlined in the E&S plan located onsite.
2177503 Primary Facility 2013-06-24 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production, brine tanks located onsite. Brine tanks appeared to be in sound condition. Drill pit located onsite and it contained drill cuttings and minimal fluids. Pit had more than the required two feet of lined freeboard. Fill and cut slopes had been temporarily stabilized and all site and access road erosion controls appeared to be functioning properly.
2235438 Primary Facility 2014-01-10 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site had been restored and the vegetative cover appeared to meet the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
