2218958 Primary Facility 2013-11-01 Routine/Complete Inspection No Violations Noted Drilling had not as the time of the inspection commenced. The E&S plan had been implemented as compiled. It was noted that to minimize the possibility of sediment generated and transported onto SR 1004 (Tidal-Widnoon Road) the entire access road and site should be rocked, and at a minimum the road. In addition the drill pit subbase was not smooth and contained many rocks and should have a dirt subbased added prior to lining and utilization of the drill pit. Contacted operator representative Ed Barret regarding these issues. Mr. Barret stated that he had already instructed the road to be re-rocked and the base of the drill pit will be brought into accordance with 25 PA Code Chapter 78.56.
2225966 Primary Facility 2013-12-03 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed, well was not equipped for production. Drill pit had ben closed and regraded. A small frac pit had been constructed and lined but did not as of yet contain any fluids. Noted tha the entire access road had been rocked. There was no observed accelerated erosion or sedimentation observed in relation to the site or access road.

Brine tank located onsite and it appeared to be in sound condition. Site regrading had not been completed.
