Well development impoundment construction was in its final stages with final grading and mulching for temporary stabilization being completed. There was no observed accelerated erosion or sedimentation in relation to the site or access road. All erosion controls had been implemented and maintained as outlined in the E&S plan contained within the ESCGP.

All access road and site erosion controls had been implemented as outlined in the E&S plan contained within the ESCGP. All erosion controls appeared to be maintained and functioning properly. The rig is in the process of exiting the site. Because of the large volume of vehicle traffic, the Department did not inspect the whole site due to safety concerns.

2709357 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted At time of inspection, well 1H drilled, not yet frac'd. Frac'g not yet completed for wells 2H, 4H, 5H, and 6H. Frac'g operations ongoing for well 4H during this inspection. Well 3H frac'd.
2721261 Primary Facility 2018-04-23 Drilling/Alteration No Violations Noted Operator had just finished fracking stage 4 of the planned 27 stages at the time of this inspection. All paperwork and equipment on site looked ok. A Scada pressure monitoring unit is set up on the offset Snyder Mushroom #1 well in order to monitor this well for potential communication later on in the frac stages. All equipment on site looked ok.

NOTE: I stopped by the Mushroom #1 offset well and spoke with the Snyder well tender who was set to monitor this well when the frac is nearest the well bore. All monitoring equipment was hooked up and functioning properly and the wellhead was in sound condition with valves and piping rated to handle the higher pressures if encountered.

2727589 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted At time of inspection, frac'g of wells 1H, 2H, 3H, 4H, 5H, and 6H have been completed. Equipment being removed from site. Operator intends to begin drilling out plugs on Monday 5/14/18.

The operator is moving equipment in to this well pad today to begin flowing back all 6 wells on this pad. The well is shut in, is tagged, the well head casing and all equipment on site looked ok.
2792877 Primary Facility 2018-10-23 Routine/Complete Inspection No Violations Noted Well appears equipped for production. Well head surrounded by cement barriers. Small generator on site located on secondary containment. Three brine tanks on site.
2821539 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The well head casing, piping, production set equipment, containment dike and tank battery look ok.
