Well development impoundment construction was in its final stages with final grading and mulching for temporary stabilization being completed. All erosion controls had been implemented and maintained as outlined in the E&S plan contained within the ESCGP.
2637395 Primary Facility 2017-09-15 Routine/Complete Inspection No Violations Noted Drilling was ongoing for 005-31296 at the time of the inspection. Secondary containment, and tertiary containment for number areas on the well pad was implemented as outlined. Well development impoundment construction had been completed and the impoundment was not lined and not in use. All access road and site erosion controls had been implemented as outlined in the E&S plan contained within the ESCGP. All erosion controls appeared to be maintained and functioning properly. There was no observed accelerated erosion or sedimentation in relation to the site or access road.

The rig is in the process of exiting the site. Because of the large volume of vehicle traffic, the Department did not inspect the whole site due to safety concerns.

2711794 Primary Facility 2018-03-28 Drilling/Alteration No Violations Noted On this date, 3/28/2018 at 3:03am the field man monitoring the Snyder Brothers Mushroom Farm #4 well noted a slight pressure increase from 876psi to 879psi. The frac crew started pumping stage # 38 of the W9 4H at 6:00am this morning. At 6:20am the pressure on the Mushroom Farm #4 well (a vertical Marcellus well) rose to 1315psi, at 6:25am the pressure rose to 1323psi and at 6:30 am it rose to 1424psi at which time the frac crew lowered their pumping rate from 98 bbl's per minute to 85 bbl's per minute. There was no improvement in pressure so they again lowered their pump rate to 70 bbl's per minute. They then cut the sand and shut down the frac at 6:45am at what will be the end of stage 38 where they have determined to be the finish of this lateral. They were going to frac 39 stages but will conclude this lateral with 38 stages. The monitored pressure on the 4 1/2" production casing of the Mushroom Farm#4 well peaked at 3320 psi at 7:05am this morning and the well head is rated for 5000 psi. As I was monito

2747770 Primary Facility 2018-07-02 Drilling/Alteration No Violations Noted On this date this well has been drilled, fracked and is awaiting flow back. The operator is moving equipment in to this well pad today to begin flowing back all 6 wells on this pad. The well is shut in, is tagged, the well head casing and all equipment on site looked ok.
