While pumping 15% HCL acid, the discharge hose off of the blender split and released ~10 barrels onto the secondary, plastic containment. Shut down operations, immediately neutralized the area and cleaned up the spill. Neutralized materail was recovered via vacuum truck and placed back into operation system to be re-used. No material was gathered for disposal. Corrective Action...replace defective hose and re-evaluate all hosed for defects. No other hoses identified as having defects and operations resumed.

Site was drilled but not equipped for production. Cellar of well had overflown from stormwater and snow melt. The overflow resulted in puddling extended approximately 30 feet from the well head. The puddle was principally light oil sheen with heavier oil sheen in its north-east quadrant. Operator began clean-up operations to remove contaminated water or soil. Department recommends Operator vigilantly maintain cellars by removing excess water before the cellar overflows. Operator should also ensure any pollutants in contact with the ground are cleaned up expeditiously.

Excess water had been removed from cellar of well. Soil showed no sheen or other visible signs of pollution. Well tag was resting on top of well and Department recommends it be re-positioned to improve visibility. Department also recommends Operator continue to maintain cellars by removing excess water before the cellar overflows.
