2565589 Primary Facility 2017-02-28 Routine/Complete Inspection No Violations Noted I conducted a routine inspection of the Pennenergy Resources, LLC B37 well pad in New Sewickley Township, Beaver County. Taylor Schwab was present from Pennenergy. I received a notification prior to this inspection that the approved modular aboveground storage structure was being erected at the site. Three wells have been drilled on this pad (2H, 3H, and 4H). The site appeared in good condition and the storage structure was being constructed consistent with the OG-71 application.

No violations.

2583336 Primary Facility 2017-04-17 Routine/Complete Inspection No Violations Noted On 4/17/2017, WQS Brian MacQuarrie onsite at unconventional well site. Three wells drilled. Secondary containment setup onsite around well and six 500 bbl frac tanks. AST setup onsite to store impaired water to aid in well stimulation activities. freshwater impoundment fenced in and maintained . Erosion and Sedimentation best management practices installed and maintained. No apparent E/S issues.
2624506 Primary Facility 2017-08-07 Routine/Complete Inspection No Violations Noted On 8/07/2017, WQS Brian MacQuarrie onsite to conduct inspection at unconventional well site. Three wells in production. Two 400 bbl production tanks installed within secondary steel containment structure. Secondary containment still setup around wells. AST onsite used to store impaired water for frac. Freshwater impoundment fenced in and maintained. Previously disturbed areas well vegetated.
2668367 Primary Facility 2017-12-06 Routine/Complete Inspection No Violations Noted Site had been constructed as outlined in the E&S plan contained within the ESCGP. All site and access road erosion controls had been implemented and properly maintained. There was no observed accelerated erosion or sedimentation in relation to the site or access road. All disturbed areas had been stabilized with vegetation attaining 70% uniform vegetative cover. Well appeared to be equipped for production. Two brine tanks located onsite within secondary containment. Both the tanks and containment appeared to be in sound condition. Constructed AST located onsite on the well pad. Well development located onsite and it appeared to contain freshwater.
2685435 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted Site had been constructed as outlined within the E&S Plan contained within the ESCGP. All site and access road erosion controls had been implemented, maintained and were functioning properly. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Well appeared to be equipped for production. There were two brine tanks located onsite within secondary containment. Both the tanks and secondary containment appeared to be in sound condition. Constructed AST was located onsite. Fluids were being loaded and trucked off-site at the time of the inspection. Well development impoundment located onsite.
