However, the site has not been restored and the pit with fluids and cuttings remains in place. Therefore, the site needs to be restored and a restoration report needs to be submitted. In addition, a well record and completion report is past due for this well. The well head needs to be tagged in accordance with § 601.201(h). A site identification sign remains in place at the entrance of the access road associated with this site. I discussed the violations with Mr. Matt Vavro on 8/19/09. I requested a schedule of implementation for resolving the issues.
