2094344 | Primary Facility | 2012-07-25 | Routine/Complete Inspection | No Violations Noted | No water in cellar. 2annular vents go to 550 gal poly tank. MSA Altair 5X gas meter reading of comb = XXX and 30% methane in tank. Sign at entrance to pad has Burlington Twp and sign on well is wrong - is for A 2H(37-015-21749). Talked to Rich Adams(Chief rep.) on 8/24/2012 to get corrected. |

No leaks detected from production equipment. Petroleum substance within 2H Well celler. No violations cited at this time. |
2717027 | Primary Facility | 2018-03-15 | Routine/Complete Inspection | No Violations Noted | Arrived at 11:00 with Jon Hafer to conduct inspection for regulatory inactive well status extension request. This is the second extension request for this well. The 9-5/8" x 5-1/2" annulus was open and vented to a 550- gallon polytank where no methane was detected. The 20" x 13-3/8" and 13-3/8" x 9-5/8" annuli could not be observed because the cellar was filled with gravel. Requested a follow- up inspection with the cellar vacced out to observe these annuli. The three other wells onsite were in production. No violations were observed at the time of this inspection. |
2718435 | Primary Facility | 2018-04-10 | Follow-up Inspection | No Violations Noted | Arrived at 09:00. Weather: cold, with snow on the ground. This was a follow up on a regulatory inactive well status extension request. This is the second extension request for this well. Jon Hafer and a Mountain Energy crew were onsite removing gravel from the well cellar to expose the 20" x 13-3/8" and 13-3/8" x 9-5/8" annuli for observation (photo 1). Annuli were filled with cement. No bubbling was observed when annuli were covered by a few inches of water. During the inspection, scorch marks were observed on one of the GPUs (photos 3 & 4). Peg Farrell responded to a query about the scorching as follows: "In February 2016, a fire was discovered to have occurred at a Kingsley A GPU, and to have been immediately extinguished upon the automatic shut-off mechanism functioning as designed. The other wells on the pad remained in operation. Since it was discovered after resolution occurred and there was only minor damage, no emergency services were engaged. No fluids were released, so there was no spill to report |
