2089196 Primary Facility 2012-08-02 Incident- Response to Accident or Event Violation(s) Noted Arrived at the site at 10:20 AM in response to a release of flowback fluid that was discovered at 10:05 on 8/1/12. Jason Petlock of SWN reported the release to the Department on 8-1-12. The initial report indicated the amount of flowback fluid released was determined to be (100-5,000) gallons. The report indicates that while SWN was in a preparation to frac a well at the site, blended water (treated flowback and freshwater from the Greenzweig large impoundment transported to the site via underground lines) overflowed the working tanks and secondary containment. The fluid exited the Well Pad and traveled under the Pad berm. SWM reported conductivity readings of (2,500) microSiemens/cm in a seep at the toe of the fill slope. A sump has been constructed in this area with a pump located on a liner to recover the fluid. During the inspection on 8-2-12, SWN indicated to the Department they have determined the out of containment and off the Well Pad release to be between 90-100 bbls (barrels) also equivalent to (4,000 to 4,200 gallons).
2100606 Primary Facility 2012-09-11 Follow-up Inspection Outstanding Violations - No Viols Req'd I used an Extech, ExStik (2) pH, Conductivity, Salinty and TDS meter to field screen areas of ponded water both on and off the Well Pad. Conductivity readings recorded on the meter indicated less than (200) microSiemens/cm on the secondary containment liner near the frac tank storing fluid from Sump#1 at the bottom of the fill slope. Conductivity readings inside the lined pit for sump# 1 indicated a range between (1,000-1,200) microSiemens/cm. In areas of ponded and slow moving water outside this sump conductivity readings revealed levels in between (800-900) microSiemens/cm. Water was observed seeping from the ground in several areas at the toe of the fill slope. The water flows into Sump#1 and is periodically pumped through a line up the embankment into a frac tank located on secondary containment on the Well Pad. Sump #2 indicated conductivity readings between (1,100-1,200) microSiemens/cm. This was inside the sump pit, which is located at the edge of the wooded area.

2343513 Primary Facility 2015-02-11 Routine/Complete Inspection No Violations Noted Arrived onsite @ 12:25, sunny, temperature > freezing, pad snow-covered. There were 6 wells in production on the pad. The Ferguson-Keisling GU #1, Heckman-Hiduk GU #1, and Heckman-Camp GU #1 were in one row. The cellars on these wells were open. There were 3 digital Scada gauges on annuli for each of these well. The lowermost labeled, “Surface”, measured the pressure in the 9-5/8” x 5-1/2” casing. Higher on the wellhead, the “Casing” gauge measured the 5-1/2 x 2-3/8” annular pressure. The gauge on the top of the well head measured the tubing pressure, according to production superintendent Gary Dutton. The production units for these wells were near the western corner of the pad. The other three wells were also in a line but offset from the previous 3: Greenzweig 7H, Greenzweig 8H, and Heckman-Hiduk #2.
2677985 Primary Facility 2017-12-05 Routine/Complete Inspection No Violations Noted On December 5, 2017, the Department conducted a routine inspection at the SWN Production Company LLC, GU-G-Pad in Herrick Township, Bradford County. The Department was represented by Andrew Hobbins. Tracy Gregory was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated, and no E & S issues were observed during the inspection. The well pad has been reduced in size. A Notice of Termination was approved by the Department in November of 2015.

Six wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and six GPUs were also on site. Puddles on the pad were low in specific conductivity and no staining was observed on the well pad surface.

No violations were observed during this inspection.

2817053 Primary Facility 2018-12-18 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Heckman Camp G pad in Herrick Twp, Bradford County on 12/18/2018 at 1230 hours. The inspection was conducted in response to a Well Site Restoration Report received by the DEP.

This site has 6 producing wells onsite with two production tanks. The site is very well vegetated and steep slopes of the pad appear stabilized. All PCSM BMPs appeared to be in place and no temporary E&S BMPs were observed.

This Well Site Restoration Report is accepted.

No violations noted today, I departed the site at 1255.
