2421228 Primary Facility 2015-10-16 Routine/Complete Inspection No Violations Noted Saxon Drilling Rig #172 is at the location drilling the Ayers 8H well at a depth of (1,379) feet. Talisman submitted a Drilling Resumption Notice to the Department on 10/13/15 indicating they would resume drilling on the 8H well at 6AM on 10/14/15. Drilling Resumption Notices were also submitted for the Ayers 2H, 4H and 6H wells over the course of the last month. These wells have most likely been drilled to total depth. The Ayers 1H well was in production, but is currently shut in. While walking around the site, I observed a small rainbow colored petroleum sheen on the ground surface near the edge of the pad on the opposite side of the pad where the pad entrance is, along with a relatively significant amount of dust blowing in the air. The dust is actually sawdust that is utilized as a drying agent mixed with drill cuttings stored in containers in the backyard.

2476303 Primary Facility 2016-04-14 Routine/Complete Inspection No Violations Noted Talisman was in the process of setting for hydraulic fracturing operations and erecting a DYNA tank, which is a temporary modular aboveground storage tank that will be utilized to store flowback and production fluid during frac operations. Talisman submitted a Request for Approval of Alternative Waste Management Practices (OG-71) application on 3/31/16. The Department acknowledged the application on 4/5/16. The production fluid is generated from other Talisman wells and transported to the site via vacuum trucks. The flowback fluid stored in the tanks could also originate from other Talisman wells and will be generated during hydraulic fracturing plug drill outs and flowback of the wells on the Ayers Pad. The 2H, 4H, 6H and 8H wells have been drilled to total depth but not yet stimulated. Frac operations are scheduled to commence on or near June 5th, according to Mr. Smith, site completions company man.
2485601 Primary Facility 2016-04-29 Follow-up Inspection No Violations Noted I conducted an inspection of the site accompanied by Andrew Hobbins, DEP O&G WQS in the Northcentral Regional Office in Williamsport, PA. There was no activity at the site during the inspection. The secondary containment structure for the DYNA tank has been completely set in place. Production and flowback fluid transported to the site from other Talisman wells pads is offloaded into (10) frac tanks inside the tank containment through an offloading manifold. The frac tanks and the (5) foot high wall made of plastic material encompassing the perimeter of the secondary containment liner for the tank, were installed in the containment sometime between 4/14/16 which is the most recent site inspection conducted by the Department, and 4/29/16. The frac tanks in the containment are relatively close to the wall nearest the site access road. Talisman has added a separate containment area in front of the wall as fluid hoses extend from vacuum trucks to the manifold and frac tanks. The containment area consists of
2485642 Primary Facility 2016-05-16 Incident- Response to Accident or Event No Violations Noted Kris Perritt, Environmental Specialist of Talisman Energy, contacted me on 5/13/16 to report a small spill of production fluid that occurred on 5/12/16. He stated the following information. "The volume of the spill is less than (5) gallons. The production fluid is generated from wells at different Talisman Pads. The spill occurred during offloading operations of the fluid. The area of the pad surface impacted is the offloading area in front of the containment wall for the DYNA tank. The impacted area will be scraped up using visual observation and soil conductivity screening on 5/13/16”.

There was no activity or personnel at the site when arrived at 11:15. I noticed there were small pockets of water on the pad surface in front and near the truck offloading area. Fluid is offloaded into frac tanks to be eventually pumped into the DYNA tank for storage until hydraulic fracturing operations commence. I recorded conductivity readings in the range of (990-1,000) microSiemens/cm³ in the water. I noticed

2493538 Primary Facility 2016-06-10 Incident- Response to Accident or Event No Violations Noted On June 10, 2016 at 1215 hrs., the Department responded to a spill notification by Talisman Energy USA Inc. (Talisman) at the Ayers (05-005) well pad. The Department was notified by Sheridan Anderson, Company man for Talisman, that approximately 93.5 barrels of production fluid was spilled when a frac tank was overfilled, causing production fluid to flow onto containment. Mr. Anderson stated they were removing the fluid from the containment with a vac truck. The production fluid will be put into another tank and used in the fracing process.

The Department arrived on site at 1215 hrs. and met with Sheridan Anderson and Dave Coxhead, Site Safety Manager for REM Security. The site of the spill was observed first. A contractor (Midosa) was at the spill site removing the production fluid from containment. A conductivity reading was taken from the remaining liquid in the containment structure and determined to be over-range of the Eutech Instruments Oakton Multi-Parameter PCTestr 35 meter that was used.

2495377 Primary Facility 2016-06-09 Incident- Response to Accident or Event No Violations Noted Sheridan Anderson, site company man, completions consultant of Talisman Energy, contacted the Department on 6/8/16 at 10:40 hours to report a hydraulic fracturing fluid spill. The PADEP Emergency Response Notification Report indicates the spill happened at 12:00 hours on 6/7/16. A fitting on a frac blender leaked resulting in a spill of (3) gallons of frac fluid to secondary containment. The material was recovered and reused in the frac process. Nothing escaped containment. No environmental impact. Frac fluid consisted of fresh water and produced water.

I arrived at the site at 11:20 AM. Halliburton is the Talisman subcontractor in the process of conducting hydraulic fracturing operations. I met with Mr. Anderson and Dave Coxhead, Site Safety Manager of REM Security. Mr. Anderson informed me the following information. "Frac operations are in progress on the Ayers 2H, 4H, 6H and 8H wells. Halliburton is on stage 4 of the 6H well. The frac started on 6/7/16. Rig up on 6/6/16. The 1H well is shut in. T

2495556 Primary Facility 2016-06-13 Incident- Response to Accident or Event No Violations Noted David Williams, Water Management Coordinator of Talisman Energy, and I arrived at 12:45 just south of the intersection of Wolf Hill and South Hill Road down gradient of the Ayers Pad, to view the impacted area of Wolf Hill Road in reference to flowback fluid spilling out of a drip pan of a vacuum truck, Fast Truck #58. Please see the Department's Talisman Upham 6-13-16 inspection report for details associated with this spill. I noticed what appeared to be staining associated with the spill on Wolf Hill Road, just south of the intersection of South Hill and Wolf Hill Road. Mr. Williams stated that sections of Wolf Hill Road in this area appears wet and darker in color due to Orwell Township conducting dust control and road stabilization operations earlier today, 6/13/16. The operation consists of spraying water out of a spreader bar on the back of a truck, onto the dirt and gravel road surface. 2504459 Primary Facility 2016-07-20 Routine/Complete Inspection No Violations Noted On July 20, 2016 at 1200 hrs., the Department conducted a routine inspection at the Talisman Energy USA Inc. (Talisman) Ayers (05-005) well pad. Andrew Hobbins, Water Quality Specialist, and Jonathan Sassi, Environmental Protection Specialist, represented the Department. Tim Dureault, Completions Foreman, represented Talisman.

Talisman is flowing back all four wells that were stimulated. They started flowing back on July 15, 2016 and will continue to do so for approximately 10 days. Two of the wells were producing 50-60 bbls. of flowback per hour, while the other two wells were producing 60-80 bbls. per hour. The flow back is being routed into the DYNA tank that is on site.

The Department conducted the post frac inspection to assess the integrity containment system and perform a conductivity screening of puddles on containment and on the surface of the well pad. 2525967 Primary Facility 2016-09-15 Follow-up Inspection No Violations Noted I arrived at the site at 12:45. I met with Jack Bolyard, well operator of Talisman Energy. Mr. Bolyard stated that the purpose of his visit to the site was to turn the wells back on line. He further stated the following information. "The wells are not on line and natural gas is not being sent to sales. The wells were in production for about 2 months. The DYNA tank was taken down roughly (3) weeks ago. The frac tanks in the tank farm in front of where the DYNA tank was staged contain production fluid. The frac tank in the center of the pad contains fluid from the sand traps as part of the wells."

I screened water for conductivity near the center of the pad, near the access road and concrete barriers in the frac tanks secondary containment, the offloading area in front of the frac tanks containment and in the containment for the frac tanks which contains fluid from the sand traps. Conductivity was near (3) milliSiemems/cm³ in the water accumulated on the pad surface, near (1) ms/cm³ in the containment

2598820 Primary Facility 2017-05-25 Routine/Complete Inspection No Violations Noted On May 25, 2017 at 1250 hrs, the Department conducted a routine inspection at the Repsol Oil & Gas USA LLC, Ayers 05-005 well site in Orwell Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the well pad had achieved greater than 70% perennial vegetation. No erosion issues were observed. PCSM has been addressed at the site.

Five wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellar. Two production tanks and five GPUs were observed in secondary containment. A frac tank was also observed in secondary containment. Puddles on the well pad surface were low in specific conductivity. No staining was observed on the well pad.

The well pad has not been reduced in size, but PCSM has been addressed at the site. A two year restoration extension has been a

2705388 Primary Facility 2018-03-12 Site Restoration No Violations Noted On March 12, 2018, the Department conducted a routine inspection at the Repsol Oil & Gas USA LLC, Ayers K 05-005 well pad in Orwell Township, Bradford County. The Department was represented by Andrew Hobbins. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was partially snow covered, but appeared to be well vegetated. No erosion issues were observed. The well pad has not been reduced in size, but PCSM has been addressed. Two infiltration basins were observed to be installed at the site to address PCSM.

Five wells were in production during the inspection. The wells were tagged and no bubbling was observed in the well cellars. Two production tanks and five GPUs were all in secondary containment. A baker tank was also on site near the well heads. The tank was in secondary containment, but the containment was filled with ice and snow.
