2618317 Primary Facility 2017-07-19 Routine/Complete Inspection No Violations Noted On July 19, 2017, the Department conducted a routine inspection at the SWN Production Company, LLC, GU-H-Pad in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. A crew was on site during the inspection working to install a compressor.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the pad were well vegetated and no erosion issues were observed. PCSM has not been addressed at the site. The site has been re-permitted as an ESCGP-2 and PCSM has been proposed to be installed this month. The area in which the bio-infiltration berm is to be installed was observed to be mowed and staked out, but has not yet been installed.

Six wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two production tanks and six GPUs were on site. No staining or puddles were observed on the pad surface. A compressor was being installed on the

2662915 Primary Facility 2017-11-14 Incident- Response to Accident or Event Violation(s) Noted On November 14, the Department conducted a incident response inspection at the SWN Production Company GU-H-Pad in Stevens Township, Bradford County. Andrew Hobbins and Jonathan Sassi represented the Department. Mike Demming was on site for SWN.

The Department was notified on 11/13/2017 by Steve Moyer, Regulatory Supervisor, that SWN discovered a spill on the GU-H-Pad. The material spilled was injection water which is a mixture of freshwater and methanol at an 8:2 ratio. The ratio of methanol to freshwater was significantly less than 8:2 which caused the injection water to freeze due to cold weather. The freezing caused a fitting to fail and injection water spilled onto secondary containment. The secondary containment around the injection water tank had a structural defect at one corner that allowed the fluid to escape and spill onto the well pad surface. Approximately 2000 gallons was released from the tank, 1500 gallons of which made it onto the well pad surface.
