2594295 Primary Facility 2017-05-15 Routine/Complete Inspection No Violations Noted On May 15, 2017, at 1150 hrs., the Department conducted a routine inspection at the Repsol Oil & Gas USA LLC (Repsol), Cook 05-040 well site in Orwell Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the well pad were well vegetated. No erosion issues were observed.

One well was in production during the inspection. The well was labeled, and no bubbling was observed in the well cellar. One GPU and two production tanks were observed to be on secondary containment. Liquid within the secondary containments was low in specific conductivity. Puddles on the pad were also low in specific conductivity. No staining was observed on the well pad surface.

The well pad has not been reduced, but PCSM has been addressed at the site. A two year restoration extension request has been approved by the Department for the Cook 05
