2088771 | Primary Facility | 2012-08-02 | Routine/Complete Inspection | Violation(s) Noted | Cellar full of water. No Bubbles. Well has been fraced. |
2194523 | Primary Facility | 2013-08-09 | Routine/Complete Inspection | Outstanding Violations - No Viols Req'd | Inspection is for 'Inactive Well Status" application. |
2372617 | Primary Facility | 2015-05-26 | Routine/Complete Inspection | No Violations Noted | Overall vegetative cover >70% with some areas of stable rock. Temporary BMPs have been removed. |
2485395 | Primary Facility | 2016-05-18 | Routine/Complete Inspection | No Violations Noted | Routine inspection to assess overall site conditions. No Chesapeake representatives were present. Partially cloudy, ~60 degrees.
Vegetative cover on site >70% and consists mainly of perennial plant species. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. A 2-year extension of time for site restoration was approved by the Department on 02/03/2016. That approval requires that post-construction storm water management (PCSM) features be installed by 05/03/2016. At the time of inspection, the PCSM features had not yet been installed. However, staking was in place at the locations where such features are to be installed. No violations were cited during this inspection. |
2561910 | Primary Facility | 2017-02-15 | Routine/Complete Inspection | No Violations Noted | No violations cited. |
2635628 | Primary Facility | 2017-09-13 | Routine/Complete Inspection | No Violations Noted | Two wells in production phase. Two each sand and water separators. No leaks. Site meets requirements for Notice of Termination(NOT) completeness review. Site recommended for NOT approval. |
2727159 | Primary Facility | 2018-05-04 | Routine/Complete Inspection | No Violations Noted | Arrived at 1414. Weather: cloudy. Two producing wells on a three slot set up. 5H has a conductor cap. Violations for defective cement were cited in 08/03/2012. No previous violations were associated with the 2H and no new violations were observed during this inspection. This inspection result is, NOVVIO. |
2815203 | Primary Facility | 2018-12-12 | Routine/Complete Inspection | No Violations Noted | Arrived onsite for a routine inspection at 11:02; the weather was ~30°F and calm. The pad had several pieces of farm equipment being stored on the eastern edge.
There were 2 properly tagged Marcellus wells staged for production. The well cellars were filled with ice; no bubbling was observed. There were 2 GPUs and sand separators onsite and in secondary containment. The cut and fill sides of the pad were stabilized with rock. The remaining 2 sides were more gently sloped and were well vegetated. Overall, the area was greater than 70% vegetative cover. No E&S issues were observed. The Department recommends repairing or replacing produced water tanks as needed. No new violations noted as a result of this inspection. |
