2635765 Primary Facility 2017-09-06 Routine/Complete Inspection No Violations Noted On September 6, 2017, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC (Repsol), Miller F 05-056 well site in Warren Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The last inspection conducted by Repsol occurred on 9/5/2017. The slopes of the pad were well vegetated. The well pad has not been reduced in size, but PCSM has been addressed. A two year restoration extension request was approved by the Department in November of 2016.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. A production tank and GPU were in separate secondary containments. Water on the pad surface and within secondary containment tested low in specific conductivity. No staining was observed on the pad surface.

No violations were observed during the ins

2815784 Primary Facility 2018-12-14 Site Restoration No Violations Noted NOT approval.

I took electrical conductivity readings from pooled water with an Oakton PCSTestr 35. A puddle on the pad surface near the production tanks returned 0.06 mS/cm. JLP
