Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chesapeake.
The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was observed to be well vegetated and no erosion issues were observed. PCSM has not been addressed at the site. Two wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Two GPUs and two production tanks were also on site. The production tanks were on secondary containment. Puddles on the pad surface were low in specific conductivity. No staining was observed on the well pad surface. The ESCGP-1 permit has expired. The well pad has not been reduced in size, and PCSM has not been addressed. The weather conditions are cloudy skies, near calm winds, with moderate to heavy rain falling, and temperatures around 70 degrees Fahrenheit. There was no activity or personnel at the site. CHK submitted a Deficient Tank Inspection Notification (Unconventional Wells) 78.57(i) for well Permit # 015-21782 to the Department on 9/6/14 at 13:14 hours. The notification and deficient tank inspection is associated with the Fisher Bra 2H well. CHK provided the following information in the tank inspection notification.
