2356429 Primary Facility 2015-03-27 Routine/Complete Inspection No Violations Noted Well has inactive status until 2019. 3 annular vents going to 3 separate poly tanks. Combustible gas readings at all annuli were 0. Gauge on top of well head read 1650 psi on 0-5000 psi gauge. Gravel-filled cellar in turn filled with water, no bubbling. Nothing else on pad.

2746336 Primary Facility 2018-06-27 Routine/Partial Inspection No Violations Noted I got electrical conductivity readings on site with an Oakton PCSTestr 35. A puddle on the pad returned a 0.35 mS/cm reading. Schlumberger on site to fracture the Marcellus well (photo 1). JLP
2761446 Primary Facility 2018-08-07 Administrative/File Review Violation(s) Noted On July 19, 2018, EOG Resources, Inc. (EOG) submitted a completion report for the Wolfe 1H gas well. The completion report documented the use of 479,070 gallons of water from the Chemung River (Barrett) operated by Chesapeake Appalachia, LLC. This source was previously authorized under the Water Management Plan, but the authorization expired September 30, 2017 and was not renewed. Information obtained via email from the Susquehanna River Basin Commission (SRBC) confirmed that unauthorized withdrawals had taken place from May through June 30, 2018, as reported by EOG to SRBC.

EOG failed to obtain authorization under an approved Water Management Plan to utilize the Chemung River (Barrett) as a source of water for drilling or hydraulic fracture stimulation for the Wolfe 1H gas well, which constitutes violations of 25 Pa. Code § 78a.69(a)(1) and Section 3211(m)(1) of the 2012 Oil & Gas Act, 58 Pa.C.S. § 3211(m)(1).

2778377 Primary Facility 2018-09-21 Routine/Partial Inspection No Violations Noted EOG hydraulically fractured the Marcellus Shale well on this pad in July, 2018.

I took electrical conductivity readings with an Oakton PCSTestr 35. A puddle on the pad surface returned a 0.18 mS/cm reading. JLP

2845706 Primary Facility 2019-02-27 Routine/Complete Inspection No Violations Noted I did not take electrical conductivity readings because i did not see any pooled liquid on site.

Air temp 16F, snowing. JLP
