2179480 Primary Facility 2013-06-19 Routine/Complete Inspection No Violations Noted Inspection for inactive status. Cellar full of water, no bubbles. 5 1/2" X 9 5/8" annulus has 15 psi and 9 5/8 X 13 3/8" annulus is vented with 0% methane and combustible.
2461447 Primary Facility 2016-03-04 Routine/Complete Inspection No Violations Noted Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. Electrical conductance was not screened due to the absence of standing water on the well pad.

Currently no active well permit exists for this facility and the site is not in compliance with the site restoration requirements as set forth in Section 3216 of the Oil and Gas Act. As of the date of this report, no request for a restoration extension has been filed with the Department. Chesapeake is reminded that a restoration request, that includes post-construction stormwater management BMPs to be implemented, should be submitted to the Department as soon as possible.

No violations were cited during this inspection.

2554000 Primary Facility 2017-01-19 Routine/Complete Inspection No Violations Noted outine inspection conducted to assess overall site conditions. No Chesapeake representatives were present.

2686374 Primary Facility 2018-01-25 Routine/Complete Inspection No Violations Noted One well in regulatory in active status. No production equipment at location. Pipeline run to location. No erosion noted. No restoration.
2725131 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted Work over rig on location. No work being performed on the 2H at this time. All materials on containment.
2727096 Primary Facility 2018-05-04 Routine/Complete Inspection No Violations Noted Arrived at 12:00. Weather: cloudy. Site muddy, no personnel on location. Rig mats down, work over rig on location. Rig drilled out plugs, being kept onsite until needed.
2837288 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 10:45; the weather was ~35°F, calm, and overcast. The pad surface was exceptionally muddy.

There was 1 properly tagged Marcellus well staged for production. The 3 cellars onsite were filled with water; no bubbling was observed. There were 2 GPUs, 2 sand separators , and 1 produced water tank onsite.

In general, the site was well vegetated. There was an area along the access road that was lacking vegetation and appeared to have been driven through (Photo #2). There were also areas near the northeast and southeast corners of the containment berm that appeared to be eroding beneath erosion blankets (Photos #3-6). The blankets were pulled taught by sediment accumulation at the bottom of the slopes. This appeared to be lifting the blankets and creating a large gap between the blanket and soil.

The Department recommends monitoring and stabilizing these areas as needed. It is recommended that an NOT and restoration report be submitted in the spring or summer if the site
