2461435 | Primary Facility | 2016-03-04 | Routine/Complete Inspection | No Violations Noted | Routine inspection conducted to assess general site conditions. No Chesapeake representatives were present.
Vegetative cover on site is greater than 70%, and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed. Electrical conductance was not screened due to the absence of standing water on the well pad. Currently no active well permit exists for this facility and the site is not in compliance with the site restoration requirements as set forth in Section 3216 of the Oil and Gas Act. As of the date of this report, no request for a restoration extension has been filed with the Department. Chesapeake is reminded that a restoration request, that includes post-construction stormwater management BMPs to be implemented, should be submitted to the Department as soon as possible. No violations were cited during this inspection. |

2686350 | Primary Facility | 2018-01-25 | Routine/Complete Inspection | No Violations Noted | One well in regulatory In-Active status. No production equipment on location. No restoration. |
2814568 | Primary Facility | 2018-12-12 | Routine/Complete Inspection | No Violations Noted | Arrived onsite for a routine inspection at 13:28; the weather was ~30°F and calm. About halfway through my inspection a white suv drove onto the pad and immediately turned around. The driver didn't stop or come very far onto the pad.
The 2H well was recently stimulated in November 2018. A large portion of the pad was still covered by containment wand rig mats. The containment showed normal signs of wear but was overall in fair condition. On containment there were 4 flowback tanks that did not appear to be plumbed to anything. There were also 6 large tanks off of containment that appeared to be empty. There was a fiberglass produced water tank onsite but not yet connected to anything. The 2H well is the only well onsite. Its cellar was empty at the time of inspection. There was no other production equipment onsite. There were some spots on the containment berm that were lacking vegetation. These areas appeared to have been temporarily stabilized and no E&S issues were observed. |
2846178 | Primary Facility | 2019-02-28 | Routine/Complete Inspection | No Violations Noted | Weather was 19 degrees and cloudy at time of inspection. Well was in production. Well had digital pressure monitor equipment attached to the production casing. The well had a 0k-5k psi analog gauge attached to the production tubing. Tubing pressure was 1600 psi. Casing pressure is 1575 psi. No violations at time of inspection. |
