Temporary erosion and sediment control BMPs have been removed. Most of the site has greater than 70% cover of perennial vegetation, though there are some patches with less and several rocky areas. No evidence of accelerated erosion or sedimentation was observed. The Department's records indicate that no current well permits exist for the site and no extension request for site restoration has been submitted or approved. In September 2015, you indicated that a restoration extension request would be submitted. To date no restoration extension request has been filed with the Department. In order to maintain compliance with the Department's regulations regarding site restorations ( Section 3216 of the 2012 Oil and Gas Act; and Title 25 Pa. Code Section 78a.65) a restoration extension request or application for a new drilling permit should be filed. No violations were cited during this inspection. |

There was 1 properly tagged Marcellus well staged for production and 2 other well cellars. All 3 cellars were filled with water; no bubbling was observed. There was 1 GPU, 1 sand separator, and 2 produced water tanks onsite and in secondary containment. One of the produced water tanks had very small amounts of corrosion near the top of the tanks (Photo #5). Both E&S and site restoration plans depicted compost filter sock sediment trap as PCSM. At the time of inspection the compost filter sock remained onsite. The pad was designed to remain at its full size. At the time of inspection the bulk of the gravel area had scattered vegetation. Areas of the containment berm around the pad edge had sparse vegetation (Photo #3). No E&S issues were observed during the inspection.
