Laddie Pendergraft, site company man and consultant of SWN accompanied me during the inspection. The current drilling depth is (856) feet. The intermediate string/section of the well is being drilled. The surface casing was cemented at (605) feet, according to Mr. Pendergraft. All other permitted wells at this location have been drilled. The Cooley 8H well is in production. Areas surveyed during the inspection included the drilling rig and associated equipment containment systems, backyard, ground surface of the Pad and production containment system and equipment. . There a few areas in the liner were patchwork was completed due to compromised liner integrity. The containment systems at the site appeared to be in good condition.

Phil Hendrix is the company man on-site. According to Mr. Hendrix, SWN is drilling the 11H well in the curve through the lateral formation with diesel based mud. The surface casing of this well has been cemented at (618) feet and the intermediate casing has been cemented at (1,603) feet. The well will be drilled to a total depth of (12,295) feet. The Cooley 1H, 2H, 4H, 5H, 6H and 8H wells are in production.

2645634 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted On October 4, 2017, the Department conducted a routine inspection at the SWN Production Company LLC, Cooley (2-Pad) in Orwell Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the pad were well vegetated. No erosion or sedimentation issues were observed. PCSM has been addressed at the site and the ESCGP has been previously terminated.

Seven wells were in production during the inspection. The wells all had the appropriate well tags and no bubbling was observed in the well cellars. One production tank and seven GPUs were also observed on secondary containment. There were no puddles or staining observed on the well pad surface.

No violations were observed during the inspection.

2664284 Primary Facility 2017-11-14 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 12:03 in response to a release of (1) to (2) barrels of freshwater mixed with methanol. I was accompanied by Andrew Hobbins, DEP O&G WQS out of the PADEP Northcentral Regional Office in Williamsport, PA. The weather conditions were cloudy skies, calm winds, with temperatures in the upper 30's. Mike Deming, Senior Regulatory Analyst and Matt Pace, Well Tender, both of SWNPC, were on-site during the inspection. Mr. Deming and Mr. Pace stated the following in reference to the release. "Due to a fitting freezing on a pump in the containment for the tank, (1) to (2) barrels of methanol mixed with water was released to the containment. There was an estimated (100) gallons of methanol in the tank and (2,000) gallons of water. The release volume to the pad surface near the 11H well, which is a separate incident from the release to containment, is believed to be less than (1) barrel, but we are not completely sure.
