Laddie Pendergraft, site company man and consultant of SWN accompanied me during the inspection. The current drilling depth is (856) feet. The intermediate string/section of the well is being drilled. The surface casing was cemented at (605) feet, according to Mr. Pendergraft. All other permitted wells at this location have been drilled. The Cooley 8H well is in production. Areas surveyed during the inspection included the drilling rig and associated equipment containment systems, backyard, ground surface of the Pad and production containment system and equipment. . There a few areas in the liner were patchwork was completed due to compromised liner integrity. The containment systems at the site appeared to be in good condition. |

The surface casing of this well has been cemented at (618) feet and the intermediate casing has been cemented at (1,603) feet. The well will be drilled to a total depth of (12,295) feet. Andrew Hobbins represented the Department. Nobody was present for SWN.
The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the pad were well vegetated. No erosion or sedimentation issues were observed. PCSM has been addressed at the site and the ESCGP has been previously terminated. Seven wells were in production during the inspection. The wells all had the appropriate well tags and no bubbling was observed in the well cellars. One production tank and seven GPUs were also observed on secondary containment. There were no puddles or staining observed on the well pad surface. No violations were observed during the inspection. |

The release volume to the pad surface near the 11H well, which is a separate incident from the release to containment, is believed to be less than (1) barrel, but we are not completely sure.
