During the inspection, Mr. Thomas informed the Department that a B&K rental forklift driven by Premium Valve Services backed into a nipple of a cam-lock fitting on a manifold of frac tanks and broke it off. Fluid then spilled out the manifold on secondary containment. The fluid pooled on the containment below the manifold, flowed into and under the frac tanks and to the corrugated pipe berm between secondary containment and the site perimeter berm. He indicated the spill occurred around 1AM and vacuum trucks arrived at the site (1.5) to (2) hours later to clean up the spill. The fluid released melted the snow on secondary containment. |

| 2665967 | Primary Facility | 2017-11-22 | Routine/Complete Inspection | No Violations Noted | On November 22, 2017, the Department conducted a routine inspection at the SWN Production Company LLC, GU-Blaine-Hoyd-M-Pad in Stevens Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN. SWN submitted a post drilling site restoration report to the Department on November 13, 2017.
The site sign and E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. Portions of the fill and cut slope were stabilized with rock. The well pad has not been reduced in size, but the operator has obtained landowner consent to allow the original footprint of the pad to remain in place. PCSM has been addressed at the site with the installation of rock infiltration trenches around the pad and along the access road. Three wells were in production during the inspection.
