2572640 Primary Facility 2017-03-17 Routine/Complete Inspection No Violations Noted On March 8, 2017, the Department conducted a routine inspection at the SWN Production Company (SWN) GU-Howland-Lent-3-Pad in Herrick Township, Bradford County. The Department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for SWN.

The E&S plan, site sign and weekly inspection logs were available for review. The slopes of the well pad were well vegetated. No erosion issues were observed. Puddles on the pad and liquid within the secondary containments were low in specific conductivity.

There were four wells in production during the inspection. The wells were labeled and no bubbling was observed. Two production tanks and four GPUs were in containment on the well pad. No staining was observed on the well pad surface.

The ESCGP-1 for the 3 Pad has expired. According to Department records, there is an active well permit (015-23284) for the 3-Pad. PCSM has not been addressed at the site. The Department recommends re-permitting the site and addressing PCSM.

2694878 Primary Facility 2018-02-12 Routine/Complete Inspection No Violations Noted On February 12, 2018, the Department conducted a routine inspection at the SWN Production Company LLC, GU-Howland-Lent-3-Pad in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was last inspected by SWN on 2/5/2018. The site was partially snow covered, but appeared to be adequately vegetated. The well pad has not been reduced in size and PCSM has not been addressed.

Four wells were in production during the inspection. All of the wells were tagged and no bubbling was observed in the well cellar. Two production tanks and four GPUs were observed in secondary containment. Water within the secondary containments was frozen so no conductivity readings were taken. No staining was observed on the pad.

No violations were observed.

2818297 Primary Facility 2018-12-17 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Howland-Lent (3 PAD) in Herrick Twp, Bradford County on 12/17/2018 at 1230 hours. The inspection was conducted in response to a well site restoration report received by the DEP.

This site does not meet the requirements for restoration under a well site restoration report. The site is not stabilized with 70% vegetative cover and temporary E&S BMPs are still installed. It is understood that Southwestern has a Consent & Order agreement with the department to install these newly permitted PCSM BMPs. Upon further discussion, the sites already inspected will have to be returned.

This well site restoration report is considered returned at this time. No violations noted today, I departed the site at 1300 hours.
