2679377 Primary Facility 2018-01-03 Routine/Complete Inspection No Violations Noted On January 3, 2018, the Department conducted a routine inspection at the SWN Production Company LLC, GU-S-Pad in Herrick Township, Bradford County. The Department was represented by Andrew Hobbins. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. SWN last inspected the site on 1/2/2018. The site was snow covered. The well pad has not been reduced in size.

Three wells were observed to be in production phase during the inspection. The wells were labeled appropriately. Another well cellar was covered with steel sheeting and had a cattle guard surrounding it. There were three GPUs on site as well as two production tanks. No staining was observed in the snow covering the pad. No conductivity readings were taken due to puddles being frozen.

No violations were observed during the inspection.

2817076 Primary Facility 2018-12-18 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Roehrig-Smith S Pad in Herrick Twp, Bradford County on 12/18/2018 at 1315 hours. The inspection was conducted in response to a well site restoration report received by the DEP.

There are currently three active wells onsite with two production tanks. Three infiltration berms/basins have recently been constructed. They appear to be constructed per the restoration plans. The areas are strawed for growth and silt sock is in place to prevent erosional discharges.

This Well Site Restoration Report is considered returned at this time. Upon stabilization with 70% perennial vegetation and removal of temporary E&S BMPs, then the report could be accepted.

No violations noted today, I departed the site at 1335 hours.
