(Phil) Hendrix is the company man on the location. The McMahon 3H well has been drilled to a total depth of (14,811) feet. The production casing of this well was cemented last night, 2/9/14, according to Mr. Hendrix. Precision Rig #539 is was in the process of being rigged down as the rig will be moving to the Greenzweig Unit F Pad to drill (3) more wells, according to Mr. Hendrix. The McMahon 1H is in the production phase of operations. The McMahon 2H-7H have been drilled to total depth, but not yet stimulated.

2627786 Primary Facility 2017-08-10 Routine/Complete Inspection No Violations Noted On August 10, 2017, the Department conducted a routine inspection at the SWN Production Company LLC, GU-VW-Pad in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Workers were on site performing maintenance work.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The slopes of the pad were stabilized with rock. Topsoil stockpiles and areas around the access road were well vegetated. No erosion issues were observed during the inspection. PCSM was addressed at the site.

Six wells were in production during the inspection. The wells were all observed to be labeled. One production tank and six GPUs were observed in secondary containment. A sand separator and frac tank were also on site and on secondary containment near the well heads. Puddles on the pad were low in specific conductivity. No staining was observed on the well pad surface.
