2223999 Primary Facility 2013-11-06 Routine/Complete Inspection No Violations Noted I approved of an alternative waste management practice (OG-71) on 10-7-13 for recovery and re-use of synthetic/oil based drilling mud and water/water based drilling mud from drill cuttings. The OG-71 application was submitted to the Department on 10/7/13. The application consisted of a revision to the drill cuttings processing and drilling mud dewatering process operated by Environmental Drilling Solutions. The revision involved the elimination of drying shakers in the process. OG-71 applications submitted to the Department for the Range Unit 21 and 62 Well sites included the use of drying shakers in the process. Drill cuttings move through a flow line and then go through primary shakers to be separated from the drilling fluid. I observed some areas of compromised integrity in the felt and the liner. The ground surface was revealed in one area with standing water on the containment near the tears. SWM informed any observable areas of comprised integrity in the containment will be repaired as soon as possible.

SDC Rig #45 is drilling the well with Diesel Oil Based mud through the curve in the Upper Marcellus Formation, with the curve almost landed. The Whipple 1H and 2H wells have been drilled to kickoff point. The 3H well has been drilled to total depth, but not yet stimulated. The Drilling Rig and associated equipment containment, which appears to be in good condition, consists of Yukon Mats made of recycled tires/rubber, wooden mats, Western Liner made of plastic material and geotextile felt set in place above and below the liner.

2564030 Primary Facility 2017-02-22 Routine/Complete Inspection No Violations Noted Arrived at the site at 11:30. I was accompanied by Andrew Hobbins, DEP O&G WQS. Halliburton is the subcontractor who will conduct hydraulic fracturing. SWN submitted a stimulation notice to the Department on 2/8/17. The notice indicated that stimulation of the 2H well would commence at 6 AM on 2/16/17. We met with Jonathan Davidson site company man of SWN Production Company LLC. Mr. Davidson stated the following. "We are in the process of rigging up for frac operations. We started on the 16th. Most of the equipment is set up. We are waiting on a few pieces of additional equipment. The 1H well is the only well scheduled to be stimulated at this time. There will be 36 stages of the frac job. The secondary containment consists of 40 or 50 mil liner, felt on top and beneath the liner, rig mats, equipment containment, duck pond containment containment trays/pans for trucks and light towers, and a 12 inch in height corrugated pipe berm for the main containment used for frac operations and around the tank
2584679 Primary Facility 2017-04-18 Routine/Complete Inspection Pending On April 19, 2017, at 1250 hrs., the Department conducted a routine inspection at the SWN Production Company, LLC (SWN)Williams-Aeppli GU-4-Pad in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Tracy Gregory of SWN, arrived on site while the Department was conducting the inspection. A representative from Erick Flowback Services was also on site.

The Department conducted the post frac inspection to assess the integrity of the containment system and to perform conductivity screenings of puddles on the well pad surface. The Department took conductivity readings from liquid within the secondary containment structure for the tank battery. All reading taken from the containment were below 600 uS/cm. The containment appeared to be in good condition and no visible holes or tears were observed. The Department did note a few areas that were identified by spray paint that had been repaired by SWN. 2637495 Primary Facility 2017-09-05 Follow-up Inspection No Violations Noted On September 5, 2017 the Department conducted a follow-up inspection at the SWN Production Company GU-4-Pad in Herrick Township, Bradford County. Andrew Hobbins an Johnathan Sassi represented the Department. Tracy Gregory represented SWN.

The Department observed the area that had elevated conductivity readings during the previous inspection. Mr. Gregory explained the remediation efforts that SWN performed to address the spill. The pad surface was scraped and two lined tri-axle dump trucks of material was disposed at the Meshoppen Railyard. Puddles that were near the previously contaminated area were low in specific conductivity.

The Department is satisfied with the cleanup and previous violations have been corrected.

I met with Tracy Gregory, Regulatory Analyst, of SWN. Flowback of the Whipple 1H, 2H and 3H wells is in progress. Natural gas is sent to the gathering pipeline and eventually to sales according to Mr. Gregory. I used a Hanna HI 9831 Soil Test EC & Temp meter, a Hanna Groline Soil Test meter and an Extech, ExStik 2 meter to record specific conductance (conductivity) of water on and off secondary containment at the site. Elevated conductivity readings were recorded on all (3) meters on individual containment structures and the pad surface near the containment for the production fluid storage tanks. Some of the readings exceeded (6) millisiemens/cm on containment. A reading greater than (4) millisiemens/cm was recorded in a small puddle of water near the site fence in between the frac tank battery and production fluid storage tank containment structure. 2747564 Primary Facility 2018-06-14 Follow-up Inspection No Violations Noted I arrived at the site at 12:36. Weather is mostly sunny, with a light breeze and temps in the low to mid 70's. An employee was in the process of spray painting the sand traps. A truck driver from Susquehanna Gas Field Services, LLC was in the process of offloading production fluid from the production fluid storage tank into a (110) barrel capacity vacuum truck labeled residual waste. I met with Mike Deming, Senior Regulatory Analyst of SWN. The Whipple 1H, 2H and 3H wells are in production. The 8H well has been permitted, but not yet drilled. The 9H well has been drilled but not yet stimulated. Flowback of the 1H, 2H and 3H wells has recently concluded. The secondary containment, gas buster tank and flowback separation units designated for flowback operations remain in place. 2748733 Primary Facility 2018-07-02 Incident- Response to Accident or Event Violation(s) Noted I arrived at the site at 11:24 in response to a reported spill of production fluid to the pad surface which occurred on 7-1-18. The weather conditions are hazy, hot and humid, with temperatures in the mid to upper 80's. I was accompanied by Ross McMann, of the Environmental Clean-up and Brownfields Program in PADEP NCRO. Mike Deming, Senior Regulatory Analyst and Ryan Klemish, Regulatory Analyst both of SWN were on-site during the inspection. I contacted Steve Moyer, Regulatory Supervisor of SWN at 8:51 AM on 7-2-18 to inquire the specifics of the spill incident. Mr. Moyer stated the following in the phone conversation. "The spill occurred as a result of a Susquehanna Gas Field Services, LLC truck driver falling asleep and not fully closing a valve on a (110) barrel capacity vacuum truck. The driver offloaded fluid from the production fluid tank. The driver reportedly overfilled the truck and fluid spilled to the pad surface. The intent was to transfer the fluid to the Zeffer Impoundment in Susquehan

2818279 Primary Facility 2018-12-18 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Whipple (4 PAD) in Herrick Twp, Bradford County on 12/18/2018 at 1125 hours. The inspection was conducted in response to a well site restoration report received by the DEP.

There are three producing wells onsite using a frac tank for produced water rather than a production tank. A residual waste truck was seen onsite offloading the tank. The small portion the the NW of the pad has recently been disturbed per plans. The area is not 70% perennial vegetative cover (see photo #4). This site does not meet the requirements for restoration under a well site restoration report. The site is not stabilized with 70% vegetative cover and temporary E&S BMPs are still installed. It is understood that Southwestern has a Consent & Order agreement with the department to install these newly permitted PCSM BMPs. Upon further discussion, the sites already inspected will have to be returned.

This well site restoration report is considered returned at this time.
