2387639 Primary Facility 2015-07-13 Incident- Response to Accident or Event No Violations Noted Justin Phillips, Health and Safety Environmental Supervisor of Southwestern Energy, reported a release of (5) barrels of production water to DEP Northcentral Regional Office in Williamsport at 19:36 on 7/11/15. The PADEP Emergency response notification report indicates that the release occurred at 18:02 on 7/11/15. While pressure testing a line during stage (5) of hydraulic fracturing conducted by Halliburton, a leak developed at a ruptured disc. As a result, (5) barrels of production water leaked onto containment. Shutdown and cleanup commenced immediately. The release occurred during stage (5) of hydraulic operations while pressure testing the Ferguson-Keisling 2H well as a result of a burst disc on the iron flow lines rupturing. Pressure levels were measured at (9,500) psi. A cap in the burst disc popped off due to mechanical malfunction releasing fluid into the air mainly in a straight direction and then to descend to the secondary containment liner and frac equipment in the center of the pad at N-41 47' 12"; W- 76 15" 10".
2403861 Primary Facility 2015-08-18 Follow-up Inspection No Violations Noted As I arrived at the site at 11:45, flowback of the 2H, 3H, 4H and 7H wells was in progress. I met with Storm Shellino, Roustabout of Southwestern Energy during the inspection. According to Mr. Shellino, flowback operations commenced on 8/13/15. All of the wells were in the process of being flowed back simultaneously and natural gas is separated from the flowback fluid and flowed through a gathering pipeline eventually to sales. Production equipment is setup. The majority of the secondary containment, which consists of geotextile felt set in place above and below a liner made of plastic material, was used during hydraulic fracturing of the wells and remains at the location.

2404705 Primary Facility 2015-09-01 Follow-up Inspection No Violations Noted I received a notification via e-mail from Afton Sterling Regulatory Supervisor of Southwestern Energy regarding the removal of secondary containment from the pad surface on 8/31/15. Flowback operations have recently concluded at the site and all wells are in production. The secondary containment was used during hydraulic fracturing and flowback operations. I had recently requested that SWN contact me to inform me as to when the containment would be removed from the site in order to determine if there was an impact to the pad surface beneath the liner associated with the flowback fluid release that occurred on 7/11/15. Please refer to the Department's 7/13/15 inspection report for additional details regarding this release. There does not appear to be any impact to the pad surface related to the release or recent activities at the site as indicated in the attached photos. There is some moisture content on the pad surface where the liner was removed.

2591913 Primary Facility 2017-05-08 Routine/Complete Inspection No Violations Noted On May 8, 2017, at 1150 hrs., the Department conducted a routine inspection at the SWN Production Company (SWN) GU-B-Pad in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. SWN maintenance personnel were on site during the inspection working on the GPUs.

The site sign, E&S plan and weekly inspection reports were on site and available for review. The site was well vegetated and no erosion issues were observed. A few areas on the fill slope could be reseeded, limed and fertilized to promote better vegetative growth. A section of the cut slope was stabilized with rock. PCSM has been addressed at the site.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. The production tank and GPUs were all on secondary containment. Puddles on the pad were low in specific conductivity. No staining was observed on the well pad surface.

2664637 Primary Facility 2017-11-15 Site Restoration No Violations Noted On November 15, 2017, the Department conducted a site restoration inspection at the SWN Production Company LLC, GU-B-Pad in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

Generally the site was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. Perennial vegetation was established throughout the site greater than 70%. No erosion or sedimentation issues were observed. The well pad has not been reduced in size, but PCSM has been addressed. Six rock infiltration trenches have been installed to address PCSM.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. One production tank and four GPUs were observed in secondary containment. Conductivity readings taken from puddles on the well pad surface were low. No staining was observed.
