2345191 Primary Facility 2015-02-17 Routine/Complete Inspection No Violations Noted Arrived on 2/17/15 @ 14:30, sunny, warmer day with snow on the pad. Met with co. man Phil Hendrix. Precison Rig 539 on location preparing to drill down to the curve and lateral using oil-based mud containing diesel. The rig just arrived from the McHanan pad and BOPs were being tested at the time of my inspection. Wooden mats were being replaced with composite mats. For this well: 13-3/8” casing set at 643’, 9-5/8” casing set at 1625’. Rig 539 to drill to 11671 TD. Once drilling is done on the Heckman Camp 4H, Rig 539 will drill the Heckman Camp 5H and 6H, also on this pad. The other wells on the pad: Ferguson-Keisling 5H, 6H, and Heckman Camp 2H are in production.

2677989 Primary Facility 2017-12-05 Routine/Complete Inspection No Violations Noted On December 5, 2017, the Department conducted a routine inspection at the SWN Production Company LLC, GU-F-Pad in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Tracy Gregory was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated. No E & S issues were observed. A Notice of Termination was approved for the site in October of 2016. PCSM has been addressed with the installation of rock infiltration trenches. The well pad has not been reduced in size.

Six wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. One production tank and six GPUs were observed on secondary containment. The puddles on the pad and liquid within secondary containment were low in specific conductivity. No staining was observed on the well pad surface.

No violations were observed during this inspection.

2727853 Primary Facility 2018-05-09 Administrative/File Review No Violations Noted The eSubmission Post Drilling Site Restoration Report was submitted to the DEP on 5/2/2018. Based on inspection report 2527725 and 2677997 the Post Drilling Restoration report was accepted on 5/9/2018.
