2593701 Primary Facility 2017-05-12 Routine/Complete Inspection No Violations Noted On May 12, 2017, at 1100 hrs., the Department conducted a routine inspection at the SWN Production Company, Greenzweig (R-Pad) Location in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. A SWN employee was on site during the inspection performing maintenance work on the GPUs.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site met the 70% perennial vegetation requirement and no erosion issues were observed. Three infiltration berms were observed to be installed.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Four GPUs and one production tank were on secondary containment. All puddles on the pad were low in specific conductivity. No staining was observed on the well pad surface.

The well pad has not been reduced in size, but PCSM has been addressed at the site.

2628622 Primary Facility 2017-08-18 Site Restoration No Violations Noted On August 18, 2017, the Department conducted a site restoration inspection at the SWN Production Company LLC, GU-R-Swisher-Pad in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present at the site for SWN. A Notice of Termination (NOT) was submitted to the Department on 8/15/2017.

The site sign, E & S plan and weekly inspection logs were on site and available for review. Generally the site was determined to meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. One production tanks and four GPUs were observed to be in secondary containment. Puddles on the pad had low specific conductivity readings. No staining was observed on the well pad surface.
