2327125 Primary Facility 2014-12-04 Routine/Complete Inspection No Violations Noted I met with Gilbert Blevins, Company Man and consultant of Chief. An Integrated Production Services (IPS) coiled tubing unit is currently on the location in the process of drilling out frac plugs on the 5H well. The current drilling depth is (8,700) feet. The unit arrived at the location on 12/3/14. Frac plugs will be drilled out on the 2H-5H wells over the course of the next week or so, according to Mr. Blevins. The 1H well is in the production phase operations. While surveying the site, I observed a few areas of compromised liner integrity and sections of the containment berm not completely secured. The containment for the light tower in the corner of the Pad where the production fluids tanks will be stored was not properly secured, along with the containment for the generator next to the Company Man’s trailer. Mr. Blevins stated to me that he would patch any holes in the containment and secure the sections of the containment berm and the containment for the light tower and generator as soon as possible.

2605849 Primary Facility 2017-06-19 Routine/Complete Inspection No Violations Noted On June 19, 2017, at 1215 hrs., the Department conducted a routine inspection at the Chief Oil & Gas LLC, Yanavitch Well Pad Location in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chief.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was observed to be well vegetated except for the areas around recently installed infiltration basins. The infiltration areas were temporarily stabilized during the inspection. Topsoil/compost has been spread on the surface of the well pad and has been vegetated. Gravel still exists in a path around the well heads and around the production equipment.

Five wells were observed to be in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Five production tanks and five GPUs were observed on secondary containment. Puddles on the well pad surface were observed to be low in specifi

2635967 Primary Facility 2017-08-29 Site Restoration No Violations Noted On August 29, 2017, the Department conducted a site restoration inspection at the Chief Oil & Gas LLC (Chief), Yanavitch Alternate Well Pad in Stevens Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chief. The Department received a Notice of Termination (NOT) for the Yanavitch Well Pad on 8/18/2017.

Generally the slopes of the well pad were well vegetated, but infiltration berm # 2 did not meet the requirements 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. A uniform, perennial 70% vegetation was not achieved around infiltration berm #2.

No violations were observed during the inspection. Based on conditions observed during the inspection, the Department recommends denying the NOT.

2776433 Primary Facility 2018-09-13 Site Restoration No Violations Noted The operator submitted a notice of termination (NOT) to the department for Yanavitch Well Pad covered under ESX12-015-0044. The department received the NOT on September 7, 2018.

Five wells have been drilled and stimulated. Each connects to one of five water separators and one of five produced water tanks. Water separator containment is felt-lined plastic and contained up to approximately one inch of water in some spots. Conductivity of water within this containment was 29.4 uS/cm. The felt portion of the containment was ripped in some places but the plastic lining was in good condition overall. Produced water tanks are in good condition and sit within a steel containment with spray-in liner. Water within this containment had a conductivity of 51.6 uS/cm. A puddle on the pad had a conductivity of 646 uS/cm.

Five well heads present each with a well head tag. Well cellars filled with water. No bubbling observed in well cellars.
