2674564 Primary Facility 2017-12-05 Routine/Complete Inspection No Violations Noted On December 5, 2017, the Department conducted a routine inspection at the SWN Production Company LLC GU-G-South-Pad in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. No E & S issues were observed. The site was well vegetated, and portions of the fill and cut slope were stabilized with rock. PCSM has been addressed at the site with the installation of rock infiltration trenches. A Notice of Termination was approved by the Department for the site in October of 2016.

Four wells were in production during the inspection. The wells were all labeled and no bubbling was observed in the well cellars. One production tank and four GPUs were observed in secondary containment. The puddles on the pad and liquid in the containment tested low for specific conductivity. No staining was observed on the well pad surface.

No violations were observed during this ins

2769095 Primary Facility 2018-08-29 Site Restoration No Violations Noted Four wells in a production phase. Four GPUs.

The eSubmission Post Drilling Site Restoration Report was submitted to the DEP on 8/28/2018. Based on this inspection the Post Drilling Restoration report was accepted on 8/29/2018. Landowner consent for restoration letter attached with report. Well pad remains original size with no reductions.
