2640519 | Primary Facility | 2017-09-27 | Routine/Complete Inspection | No Violations Noted | Three wells drilled. Production equipment on location. Flowback operations appear to be on going. Flowback going into one baker tank. No leaks detected. Plastic liner and felt backer remain around well heads. |
2670858 | Primary Facility | 2017-11-30 | Routine/Complete Inspection | No Violations Noted | I arrived at the site at 13:14. Weather conditions were mostly sunny and breezy with temperatures in low to mid 40's. I met with Rob Dale, well tester of Oil States International. Mr. Dale informed me of the following regarding site activities. "We just finish rigging down flowback operations and are picking up equipment. Flowback of the wells finished at 10 AM today. We started on 11/24/17. The 2H and 3H were wells were flowed back. The 1H was in production but it is currently shut in. Natural gas was sent to the gathering pipeline and then to sales during the flowback. The 2H and 3H wells are currently in production. Fluid and sand in the traps was transferred into a frac tank next to the tank battery. Flowback fluid was transferred to the frac tanks in the tank battery. The frac tanks have been here since the frac. Vacuum tracks take the fluid out of the tanks to the Chief Beirne Green Hills Farm Pad where stimulation (hydraulic fracturing) is in progress." I left the site at 14:08.

2719505 | Primary Facility | 2018-03-30 | Routine/Complete Inspection | No Violations Noted | I arrived at the site at 12:42. The weather conditions were cloudy skies, light winds with temperatures in the mid to upper 40's. There was no personnel or activity at the site. The Ambrosius Unit A 1H-3H wells are in the production phase of operations. I observed trash scattered on the ground surface at the site. Trash was observed on the ground surface at the site during an inspection conducted by the Department on 11/30/17, inspection ID # 2670869. I performed field screening for specific conductance (conductivity) in water on the pad surface, in water in secondary containment on the pad surface, in the site perimeter stormwater ditch at the base of the site cut slope and top of the site fill slope, sediment trap on the fill slope, and production fluid tanks and natural gas production units containment structures. The conductivity was elevated in water in the production fluid storage tanks containment structure. The elevated reading indicates the potential of a production fluid spilled to the water in the |
2824004 | Primary Facility | 2019-01-08 | Routine/Complete Inspection | No Violations Noted | Arrived onsite for a routine inspection at 12:08; the weather was ~37°F and calm.
There were 3 properly tagged Marcellus wells staged for production. All 3 well cellars were filled with gravel. There were also 3 GPUs and 3 produced water tanks in secondary containment. The areas around the pad were either greater than 70% vegetative cover or rock lined. No E&S issues were observed. The infiltration basin was holding a large volume of water (Photo #1). It's possible this is due to the large amounts of rain over the past several months. It may be advisable to monitor the basin as weather permits to determine if any maintenance is needed. It appears that a 2-year restoration extension was granted on 2/13/18. No violations noted. |
