2668907 Primary Facility 2017-11-29 Routine/Partial Inspection No Violations Noted At the time of inspection, no activity was taking place on the pad. Gate at the front was locked. Well is temporarily shut in while the B 6H well is drilled. Well is surrounded by a cage. I did not see a well tag on well. XTO was made aware of this and will be taken care of. There is a line heater within a lined, steel dike and 2 tanks contained within a separate steel dike. A gas buster is also onsite. Emergency Response sign remains posted.
2693179 Primary Facility 2018-02-12 Routine/Partial Inspection No Violations Noted On 2/12/2018, WQS Brian MacQuarrie onsite. Terry Bowser construction crew onsite doing post drilling cleanup activities. Drill Rig mobilized off of site on Saturday. Two wells drilled (Permit Nos.37-019-22473 and 37-019-22694). Five roll off boxes and one Frac tank onsite for temporary disposal of residual waste. Two 400 bbl production tanks installed within secondary containment. No apparent issues. Snow on ground.
2741823 Primary Facility 2018-06-13 Routine/Partial Inspection No Violations Noted On 6/13/2018, WQS Brian MacQuarrie onsite. Well stimulation activities completed at the 6H, Permit No. 37-019-22694. Two wells now onsite and in production, 4H-37-019-22473 and 6H- 37-019-22694). Rolloff boxes and Frac tanks onsite for temporary disposal of residual waste awaiting transportation to appropriate disposal facility. Tank battery consists of three 400 bbl tanks installed within containment. Previously disturbed areas well vegetated.
2783326 Primary Facility 2018-09-26 Complaint Inspection No Violations Noted On site concerning a report of excessive noise coming from the Christensen well pad. Inspection shows no drilling or stimulation equipment on site. Wells at time of inspection were not in production. Spoke with Gary Clark and he mentioned that there are waiting for Acoustic Sounds to set up a 3 day monitoring system test to see if a sound walls are needed and if so a proper design. I have requested the production Forman Casey Eckman (412-585-2815) to notify when the test is scheduled to begin.
2799365 Primary Facility 2018-11-06 Complaint Inspection No Violations Noted On site for follow-up inspection concerning a report of excessive noise coming from the Christensen well pad reported on 9/25/18. Inspection shows two wells on site the Christensen Unit 4H and the Christensen 6H. At the time of the inspection the Christensen Unit 4H was in production with the Christensen 6H shut in. Two designated GPU and dryers for each well (photo attached). Previous report explained a three day test was scheduled to see if sound walls were needed. I spoke with production Forman Casey Eckman (412-585-2815) by phone concerning the status. The test was not conducted but sound walls are going to be installed. Production could be heard flowing through the unit upon entering the well pad approximately 100 yards + or - away. At the same time traffic could be heard behind me on Saxonburg Road. Freshly planted trees (photos attached) have been planted along the lease entrance and along the side of the location with the production units and tank battery.
2840068 Primary Facility 2019-02-13 Complaint Inspection No Violations Noted On site for follow-up inspection for report of excessive noise coming from the Christensen 4H Well Pad, 1st reported on 8/2/2016 and again on 9/25/2018. Inspection shows the Christensen Unit 4H (019-22473) in production with GPU & in-heater active at time of inspection. The Christensen 6H is currently waiting to be cleaned out so flow back can occur. Six 500bbl tanks in containment on site pictured in photo 3. Sound walls have been installed around the production units and in line heaters pictured in photo's 1 & 2. At time of inspection production could not be heard flowing through the GPU and heater at the entrance of the well pad.

tied in for production and the Briston Unit 5H (019-22696) equipped with a frac head as reported on the 11/6/18 inspection report (Insp Record 2799399) and pictured in photo 1.
