2664008 Primary Facility 2017-11-16 Routine/Partial Inspection No Violations Noted At the time of inspection, a Soilmec SR 30 auger rig is onsite drilling mouse holes. Well was spud and has 24" conductor set to 42'. Cellar is also set. Well was covered with a steel plate. Cuttings were being sent to a roll off. There were sound walls set up on the eastern and northern sides of the pad. Location is stoned. The Emergency Response sign and permit sign are posted at the entrance to the lease access road.
2671858 Primary Facility 2017-12-07 Drilling/Alteration No Violations Noted At the time of inspection, Appalachian Drillers rig 1 was onsite drilling 12 3/8" hole. Surface casing cemented using 530 sxs Class A (115 bbls), 2% Gypsum, 2% CaCl2, 15.6 ppg, 1.22 yield. Displaced with 69 bbls water and landed plug, floats held. Did not get cement to surface. DEP notified with top of cement believed to be 40' down. This was grouted to surface. A float collar and float shoe were used with 4 centralizers. Bulk of water encountered during drilling was at 170', it was fresh. Plan is to drill to 2066' and set 2046' of 9 5/8" casing. Containment appeared in good condition. Chemicals were all on it.
2671924 Primary Facility 2017-12-11 Routine/Partial Inspection No Violations Noted At the time of inspection, Appalachian Drillers rig 1 has moved offsite and the last of the equipment is being removed. Intermediate casing was cemented using 700 sxs Class A (152 bbls), 2% Gypsum, 2% CaCl2, 15.6 ppg, 1.22 yield. Displaced with 156 bbls water and landed plug, floats held. 21 bbls cement was returned to surface. A float collar and float shoe were used with 16 centralizers. Containment remains laid out and is being held down by bags of pellets. Well is covered by a steel plate and surrounded by a cattle fence.

There was a good deal of rain water within containment. All chemicals were within containment. One of the berms near the majority of chemicals was torn and ripped apart. I told the company man about this and he said that he already had a containment guy coming out today to fix.

This was displaced with 318 bbls water. Plug landed and 14 bbls of spacer returned to surface. Floats held.

Tank battery consists of three 400 bbl tanks installed within steel secondary containment structure. Infiltration berms and basin installed. Major modification to address infiltration basin submitted by the operator to the Department. Vegetative growth established in previously disturbed areas.
2813187 Primary Facility 2018-12-10 Routine/Partial Inspection No Violations Noted On 12/10/2018, WQS Brian MacQuarrie conducted a site inspection. Site consists of two wells in production the 1H-019-22645 and 2H -019-22646). Production equipment consists of three 400 bbl tanks, 2 GPus and TVO. A Well Site Restoration Extension request was submitted by the operator on 8/31/2018 for the purpose of addressing stormwater issues. A major modification has been submitted and extension will be granted until 8/30/2019.
