This well had 16" set to 42' with 2 centralizers within a 20" hole. The cellar has been set in well. Cement not poured yet. The cuttings are being moved to the area where the freshwater impoundment is being built. This has not been lined yet. The impoundment is fenced in. Posts are set around pad, but no fencing yet. The Emergency Response sign and permit are posted at the entrance to the access road.
2659538 Primary Facility 2017-11-08 Routine/Partial Inspection No Violations Noted At the time of inspection, Highlands rig 9 is over the #2H well. Surface casing for this well was cemented using 370 sxs (80 bbls) Class H, 2% Gypsum, 2% CaCl2, 5% salt, 15.8 ppg, 1.22 yield. Plug bumped and floats held. 20 bbls cement returned to surface.

Intermediate casing was cemented using 685 sxs (146 bbls) Class H, 2% Gypsum, 2% CaCl2, 5% salt, 15.8 ppg, 1.2 yield. Plug bumped and floats held. 20 bbls cement returned to surface. A float shoe and float collar were used for both strings of casing. Surface casing had 8 centralizers and intermediate had 33 centralizers installed. Both had good cement jobs per Co-man.

This was done using a lead 1 of 260 sxs HalCem (52 bbls), 14.5 ppg, 1.132 yield, a lead 2 of 720 sxs ShaleCem (145 bbls), 14.5 ppg, 1.135 yield, and tail of 570 sxs SoluCem (184 bbls), 15.2 ppg, 1.819 yield. This as displaced with 20 bbls KCl, followed by 225 bbls freshwater. Plug was bumped and floats held (3 bbls back). 24 bbls cement were returned to surface.

2747366 Primary Facility 2018-06-29 Incident- Response to Accident or Event Violation(s) Noted On 6/28/2018, at approximately 4:00 PM a release of greater than 1 bbl of produced fluids to the well pad occurred while conducting well stimulation activities. Notification was provided to the Department at 4:58 PM on 6/28/2018. The release occurred while utilizing a filter pod system. The filter pod drain plug and valve were not closed and put in place resulting in the overflow of produced water out of containment and onto the gravel well pad. Crews immediately utilized onsite vacuum equipment and absorbent material. Nitro Excavating Services was mobilized onsite and impacted gravel was placed/staged on plastic and in roll-off container for disposal.

1/2/19 CACP addressing these violations was sent out.

On 6/29/2018, WQS Brian MacQuarrie was onsite and met with Joe O'Connor -Penn Energy to investigate incident. Excavation material placed in roll-off and staged within containment. Excavation backfilled.
