2647384 Primary Facility 2017-10-10 Routine/Partial Inspection No Violations Noted At the time of inspection, HAD was onsite with 2 rotary rigs drilling conductor on the #4MH and #8MH wells. Well has conductor set to 98' GL. Cuttings were being sent to closed top tanks. Manholes on top were left open to vent because air was being used to drill. Rig and air packs were on containment. Cellars have not been set yet. Permits were in Company man's truck. The Emergency Response sign is accurately posted. Permit sign does not have all wells on pad posted. A new sign was going to be made. Copies of permits will be placed in mailbox in the meantime.
2662203 Primary Facility 2017-11-08 Routine/Partial Inspection No Violations Noted At the time of inspection, Universal was onsite and just finished up cementing surface casing. 545 sxs Type 1 (116 bbls), 3% CaCl2, 15.7 ppg, 1.2 yield. Displaced with 97 bbls water and bumped plug. 25 bbls cement returned to surface. Floats held. A float shoe and float collar were used along with 9 centralizers. Did not see any issues associated with cement job.
2669173 Primary Facility 2017-11-30 Routine/Partial Inspection No Violations Noted At the time of inspection, H & P 521 has just skid over to the 3MH well. This well had intermediate casing cemented using a lead of 480 sxs Gas Block (133 bbls), 2% CaCl2, 14.5 ppg, 1.55 yield and tail of 183 sxs Gas Block (42 bbls), 2% CaCl2, 15.7 ppg, 1.29 yield. This was displaced with 187 bbls water. Plug landed. Float held. 20 bbls cement back to surface. A guide shoe and float collar were used, along with 18 centralizers.

This well had production casing cemented using a lead of 405 sxs GasStop, 14.5 ppg, 1.431 yield and tail of 2110 sxs ShaleCem, 15 ppg, 1.267 yield. Displacement was 312 bbls freshwater. Plug was bumped and floats held. 100 bbls spacer to surface. Top of cement estimated to be at 1500'.

2749678 Primary Facility 2018-07-05 Incident- Response to Accident or Event Pending Inspected location for a follow up of a site fire. Fire was reported to 911 at 11:50 am and to the department at by Edge Marc Engery ( EM) representative Jeff Geardoff @ 13:30 7/4/18.

On site at the time of the inspection EM representatives Will Walsh and Ben Reither assisted inspector Travis Walker and myself. EM rep Walsh explained the operations at the time of the fire. They were in the process of flowing back all 10 wells at the same time after completion. At approx 11 am on July 4, 2018 they started flowing oil out of the separators into the 500 bbl frac tank #3. The separators on location had approx 60 bbl of oil in their tanks. At that level the operation is to reduce the level by pumping oil into one of the 3 oil tanks. At the time of the inspection it is unknown if all 3 tanks are tied together or are seperate containers. Also frac flow back water had been being pumped into 5 - 500 bbl frac tanks through an hdpe manifold. These 5 tanks have no check valves and were 3/4 full.
