2647391 Primary Facility 2017-10-10 Drilling/Alteration No Violations Noted At the time of inspection, HAD was onsite with 2 rotary rigs drilling conductor on the #4MH and #8MH wells. Conductor has been set to 70' so far. Cuttings were being sent to closed top tanks. Manholes on top were left open to vent because air was being used to drill. Rig and air packs were on containment. Cellars have not been set yet. Permits were in Company man's truck. The Emergency Response sign is accurately posted. Permit sign does not have all wells on pad posted. A new sign was going to be made. Copies of permits will be placed in mailbox in the meantime.
2650359 Primary Facility 2017-10-12 Routine/Partial Inspection No Violations Noted At the time of inspection, well has been spud. 20" Conductor pipe was set to 98' GL. Cellar has not been started yet, but should start setting today. Currently, HAD was onsite finishing drilling mouse holes.
2662744 Primary Facility 2017-11-14 Routine/Partial Inspection No Violations Noted At the time of inspection, H & P 521 is over the 10 MH well. This well had surface casing cemented using 550 sxs Type 1 (118 bbls), 3% CaCl2, 15.6 ppg, 1.2 yield. Displaced with 98 bbls water and bumped plug. 25 bbls cement returned to surface. Floats held. A float shoe and float collar were used along with 9 centralizers. Rig was skid over to the 6 MH from here.
2670240 Primary Facility 2017-12-05 Routine/Partial Inspection No Violations Noted At the time of inspection, H & P 521 is working over the 5 MH well. This well had intermediate casing cemented using a lead of 507 sxs Gas Block (140 bbls), 2% CaCl2, 14.5 ppg, 1.55 yield and tail of 186 sxs Gas Block (43 bbls), 2% CaCl2, 15.7 ppg, 1.29 yield. This was displaced with 192 bbls water. Plug did not bump. Float held (1/2 bbl bled back). 22 bbls cement back to surface. A guide shoe and float collar were used, along with 28 centralizers.

2687368 Primary Facility 2018-01-25 Drilling/Alteration No Violations Noted At the time of inspection, rig was waiting for Halliburton to arrive. They plan to be cementing by 7:30 PM. Containment appeared to be in good shape. 5 1/2" was set with a float shoe, Debris (float) collar, latch down float collar and two Orio toe sleeves. There were also 166 centralizers used.
2690708 Primary Facility 2018-01-30 Routine/Partial Inspection No Violations Noted At the time of inspection, H & P 521 was on the 2 MH well. The 4 MH had production casing cemented using a lead of 405 sxs GasStop, 14.5 ppg, 1.43 yield and tail of 2580 sxs ShaleCem, 15 ppg, 1.27 yield. This was displaced with 363 bbls freshwater. Landed plug and pressured up to 3540 psi. Floats held. 20 bbls spacer returned to surface and top of cement is estimated to be at 2600'.
