2687366 | Primary Facility | 2018-01-25 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, HAD was onsite with a dual rotary rig preparing to spud the 1MH well. This well had 20" set to 98' and has been cemented in. Cellar was set. Cuttings are being sent to a tank. The Emergency Response sign and permit sign are posted at the entrance to the access road. Location is stoned. Surrounding areas where earthwork took place have been seeded and mulched. |

2742107 | Primary Facility | 2018-06-14 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, no activity was taking place at the wells. A notification was given that this well would be plugged today. Steel grated plates are covering the well cellar and there are plastic jersey barriers surrounding the well. Cellars are full of water. Spoke to the company man for the site and was told that management requested the plugging to be rescheduled until next week. The Notice of Intent to plug is good for 30 days from the time it was received, but a notification (phone or, preferably, email) will need to be given when plugging will begin. |
2744056 | Primary Facility | 2018-06-20 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | At the time of inspection, conductor was to be filled with gravel and capped. The top 2' of conductor has already been cut off. The cellars and wells were sucked dry yesterday. A water truck was still onsite sucking water out of containment. I was onsite to witness the 8MH and 6MH get plugged, but left because truck had to leave to get another load of stone. The stone was being poured into the well starting at 9:20 AM.
I called company man at 1:30 PM and all conductors and mouse holes were filled in and steel caps welded on top. |
