Well head consisted of 8¿ pipe reduced to 4¿ reducer to 2¿ production line with multiple shut off valves. Production equipment consisted of data logger (10/5 psi), separator and 100 bbl brine tank (< 10 bbl fluid). Outside of brine tank heavily stained from former overspills. No signs of leakage of gas around wellhead. At the time of inspection, no E&S issues and possible IW issues with brine tank.
2269960 Primary Facility 2014-05-13 Administrative/File Review In Compliance with Policy Admin Inspection in response to a report of a brine tank spill. Spill discovered morning of 05/13/14, operator suspects leak began previous week when similar event occured. Operator claims act of vandalism. Larry Fetterman of Alliance Petroleum states approx. 30 barrels of brine tank content were releases due to valve being opened and plug pulled. Operator reminded of spill policy.Larry reported signs of distressed vegetation near tank. Operator reminded to keep in contact with Dept regarding remediation plans and progress. Violations pending further information gathering and a field inspection.
2271186 Primary Facility 2014-05-14 Incident- Response to Accident or Event Violation(s) Noted Inspt in response to admin inspt 2269960 for reportable release of brine.At time of inspection, Opr rep Larry Fetterman on-site.Per Mr. Fetterman, release occurred as result of vandals removing & stealing plug off valve on BT & cracking tank above valve.Tank had 40 bbl brine; at time of spill discovery, 10 bbl brine remained in tank, hence Mr. Fetterman stated vandals closed valve before remaining tank contents could be released.Opr believes this incident occurred on 5/5/2014 as another tank in area was reported as vandalized at that time (record 2269597).Dead veg extends from BT approx 30 ft down hillside in 2 directions.Utilizing range-finder, down-gradient pond appears to be located approx 29 yds from tank.Opr plans to sample pond.Excavation of impacted materials scheduled for 5/15/14).Dept recom'd Opr install E&S BMPs prior to excavation.Per spill policy, Opr to utilize the attainment method of alternative remediation (background/SHS).

2554709 Primary Facility 2017-01-12 Routine/Partial Inspection Violation(s) Noted Routine partial inspection conducted to assess site remediation for a reportable release of brine at this site. Violation was issued under inspection record #2271186 and corrected under inspection record #2393101 on 7-22-15. This release occurred while the well was in production. The Operator was previously In Compliance with Policy since Operator appeared to be following procedures outlined in the Department’s document: Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads, (Doc. #800-5000-001) by submitting a 15-day response on 5-23-14 and a SCR on 5-21-14. SCR was approved by ECB on 6-30-15. At the time the violation issued for the release was closed, the impacted soils had been excavated and removed from the site. The brine tank had been removed from the site.

At the time of this inspection, the area where the brine tank was located is not vegetated. The area of excavation does not appear to be backfilled therefore, the area previously impacted by the release does not appear to be res

2569055 Primary Facility 2017-03-08 Administrative/File Review Outstanding Violations - Viols Req'd Admin inspection to inspection record #2554709 where violations were issued for failure to restore disturbed area of a well site and for failure to have site permanently stabilized and protected from accelerated erosion and sedimentation upon final completion of earth disturbance activity.

Department received response to NOV from Operator. In this response, Operator states that the previously excavated area will be backfilled, fertilized, seeded and mulched as soon as weather permits.

A follow up inspection will be conducted at a later date.

Previously issued violations still exist for this site.

2580539 Primary Facility 2017-04-03 Follow-up Inspection Outstanding Violations - Viols Req'd Compliance Evaluation Inspection to inspection record #2554709 where violations were issued for failure to restore disturbed area of a well site and for failure to have site permanently stabilized and protected from accelerated erosion and sedimentation upon final completion of earth disturbance activity.

At time of inspection no E&S issues observed on access road or on site. It appears site has been backfilled, seeded, and mulched. Compost filter sock installed down gradient of backfilled area. Department recommends Operator remove temporary BMP's when site has reached permanent stabilization. Previously issued violations still exist for this site due to the lack of vegetation on previously disturbed area and the site failing to reach permanent stabilization. A follow up inspection will be conducted at a later date.

2586393 Primary Facility 2017-04-24 Compliance Evaluation No Violations Noted Compliance Evaluation Inspection to inspection record #2554709 where violations were issued for failure to restore disturbed area of a well site and for failure to have site permanently stabilized and protected from accelerated erosion and sedimentation upon final completion of earth disturbance activity.

At time of inspection no E&S issues observed on access road or on site. Previous inspection (record #2580539) indicated that site was seeded and mulched. At the time of this inspection vegetation is growing throughout backfilled area. Vegetation has not yet reached >70% uniform growth, however, Operator appears to be in compliance with schedule at this time. Department recommends Operator remove temporary BMP's when site has reached permanent stabilization. Previously issued violations under inspection record #2554709 have been corrected.

2626301 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted Well appears to be in production. A reportable release of brine occurred at this site on 5-14-14 and the related violation was closed on 7-22-15. A violation was issued on 1-11-17 for failure to restore the site after the above mentioned reportable release. That violation was closed on 4-24-17 due to operator being In Compliance with Schedule. At the time of this inspection site does not appear to be permanently stabilized and does not appear to be restored. Department recommends operator reseed and mulch to ensure site reaches permanent stabilization. A follow up inspection will be conducted at a later date.
