1922805 Primary Facility 2010-10-19 Routine/Complete Inspection No Violations Noted During this inspection met onsite with PNG Rep Paul Sekinger and driller. Sign and ES plan present, well staked not yet drilled. Filter fence installed surrounding perimeter of pad. Operator, contractor, and driller reviewed site and pit construction and agreed (recommended) that pit slopes be regraded in order to ensure adequate freeboard can be maintained and drilling pipe can be installed properly over pit. It was also cooperatively agreed that entire pad could be lowered in order to achieve a stable grade. Dept also recommended that operator reinforce filter fence with haybales and temporarily seed and mulch in order to sustain stability during cold months. The pad is relatively flat an accelerated runoff would appear to be minimal. Access road entrance sufficiently rocked. DEPT initialed and dated various changes noted on ES plan. A follow up inspection will be conducted early next wk to observe progression of mutual recommendations.
1922807 Primary Facility 2010-10-27 Routine/Complete Inspection No Violations Noted Well staked, not yet drilled. Site properly signed with ES plan. Access road and entrance stable. Unlined pits appear structurally stable. Pad is adequately rocked. Hay bale reinforced filter fence installed down gradient of pad and filter fence surrounding perimeter. Observed the installation of vegetative matting in ditches. Recommend that additional rock/ditches be added to left of access road entrance to eliminate ponding water as well as behind sediment trap. Left message with PNG Paul Sekinger and later spoke to Wayne Eremic of Plum Contracting regarding concerns and recommended corrective measure to help eliminate the possibility for a pollution event. Site had been recently seeded and mulched. No accelerated ES issues noted.
1925573 Primary Facility 2010-11-04 Drilling/Alteration No Violations Noted At the time of inspection, no E&S and IW issues. PNG contracted Union to complete storage well. Mark Shoemaker present and reviewed site status with DEP. Union started on 11/4/10 and ran 20¿ conductor to 42 ft. with grout mix. Union was completing the 17 ½¿ hole to roughly 144 ft. to set 13 3/8¿ pipe and set upper zone conductor to prevent upper rock stability. Filter fence was installed. Pit covered with 20 mil plastic with discharge line from rig to pit secured with cement blocks. No freeboard issues.
1927062 Primary Facility 2010-11-10 Routine/Complete Inspection No Violations Noted At the time of inspection, well site properly signed w/ ES plan. Rig onsite actively drilling. Access road adequately rocked. Two below grade pits lined (30mil according to PNG) and sustained adequate freeboard. Operator utilizing drilling pipe with diffuser, as well as continually washing cuttings off sides of pits to ensure freeboard levels are not compromised. Reinforced filter fence installed down gradient from pad and functioning. Recommend that operator add additional rock to vegetative ditches as growth has not yet been observed, in order to resist possible erosion. No accelerated erosion or sedimentation issues noted. Operator should continue to monitor all bmps until final restoration is complete.
1929838 Primary Facility 2010-11-23 Drilling/Alteration No Violations Noted At the time of inspection, no E&S and IW issues. PNG contracted Union to complete storage well. 1967669 Primary Facility 2011-04-19 Routine/Complete Inspection Violation(s) Noted At the time of inspection, Site Id sign and ES plan present. Well drilled not yet in production. Properly lined (30 ml) below grade pit located onsite with cuttings and minimal fluid present, recommend operator re-secure orange fencing to prevent third party access, as it has fallen down. Operator has plans to dispose of all pit contents off site. BMP¿s installed according to Es plan, but observed that (temporary bmp¿s) hay reinforced filter fence below drill pit are compromised and not functioning properly (specifically FF over topped with water and sediment- see pictures). Also noted significant ponding of water along left side access road near pipeline project and additional over topped filter fence. Due to lack of maintenance of bmp¿s a violation is being issued. Department has plans to conduct onsite meeting with operator to discuss ES violations and corrective actions.

2002466 Primary Facility 2011-09-01 Routine/Complete Inspection Violation(s) Noted At the time of inspection, Site Id sign and ES plan present. Access road and pad adequately rocked. Site has been recently seeded and mulched but <70% uniform vegetative growth observed. According to WRAC records, drilling completed on 12/18/11; therefore, nine month permanent restoration deadline is 9/18/11. Bmp¿s installed around perimeter of site. Filter fence along roadway leading to site entrance is structurally compromised by accelerated erosion and sedimentation; therefore, BMP is improperly functioning allowing water and sediment to discharge onto roadway outside of limits of disturbance. A violation is being issued for operator¿s failure to maintain BMPs in order to minimize the potential for accelerated ES. A follow up inspection will be conducted in near future.

2010515 Primary Facility 2011-10-06 Compliance Evaluation Outstanding Violations - No Viols Req'd CEI inspection with Paul Sekinger, Ernie Moses from PNG as well WQS Supervisor Ed Ritzer, WQS Marc Maestra, and myself. At the time of inspection, it appears that operator has restored previously compromised bmp¿s hence responding to bmp violation issued on 9-1-11. Although the site has been seeded and mulched, approximately 70% uniform vegetative cover, previously identified areas of concern appear to still be inundated with water. Thru a discussion, PNG and DEPT agreed to allow operator to plan a wetland seed mixture, in addition to allowing site to achieved uniform 70% permanent vegetative cover capable of resisting erosion. At this time, site appears to exhibit 50-60% uniform vegetative cover. Therefore, during this inspection one previously issued violation is corrected, yet there are two outstanding violations that operator appears to be actively addressing. No new violations noted.
2017462 Primary Facility 2011-11-02 Compliance Evaluation Outstanding Violations - No Viols Req'd well sign and ES plan present. Access road stable. PNG reps, Paul Sekinger ,Ernie Moses present during inspection. Site regraded and approximately 70% uniform vegetative cover observed. Since last inspection, operator has been unsuccessful in redirecting and/or mitigating ponding water located below gradient from site. It appears that ponding water is a result of runoff from back side of well pad, recommended that operator install a rock lined ditch would could direct/control the runoff.At last inspection, it was noted that operator would try to plant a "wetland" seed mix in inundated area, but during this inspection, department observed approximately three cattails, little evidence of additional seed mix. At this time, no new violations are noted, but outstanding violations remain. Although temporary bmp's are in place, it appears that direct runoff from the pad is a factor in the ponding water below the site.

2018699 Primary Facility 2011-11-14 Compliance Evaluation No Violations Noted Since last inspection, it appears operator has been successful in redirecting and/or mitigating ponding water located below gradient from site by adding additional rock to pad. Although ponding of water is not observed, it is recommended that operator reseed/mulch area previously impacted by runoff, as there is no evidence of growth at this time. At last inspection, it was noted that operator would try to plant a "wetland" seed mix in inundated area, during this inspection, department observes cattails. Although temporary bmp's are in place and appear to be maintained , it appears that direct runoff from the pad was a factor in the ponding water located below the site and the addition of rock may have minimized the runoff and reduced the ponding water; therefore, it is recommended that operator continue to closely monitor all bmp's as well as the direction of surface runoff from pad until final restoration achieved. approximately 70% veg cover, although some areas appear they may need reseed to sustain uniform coverage capable of resisint eros
2022145 Primary Facility 2011-09-14 Follow-up Inspection Outstanding Violations - No Viols Req'd meeting requested by the department with PNG (Paul Sekinger) to discuss violations noted on 9-1-11 inspection¿partial inspection. Present from the DEP: myself, WQS Supervisor Ed Ritzer, and WQS Marc Maestra; PNG Reps: Paul Sekinger(Consulting Engineer), Kurt Hundertmark(Manager Ops), Ernie Moses(Sr. Engineer), Joyce Dailey(Legal Counsel), and Missy Galovich(Well Tender). During this meeting, attention brought to ES controls below drill pit, as well as lack of seed and mulch along sides of fill slope of pit. Although recent heavy rains occurred, the department notified PNG on 9-2-111 of ES control failures, Department reminded operator that it is their responsibility to inspect BMPs until final restoration of site is achieved, especially after every rainfall event to ensure proper functioning ability of BMPs. ES filter fence issues addressed, sediment still noted outside fence. Pond water has developed result of regrading and pad construction. Add addtional seed/mulch to aid in control of runoff--possbible wetland mix and addtional rock to pad.
