2501258 | Primary Facility | 2016-06-14 | Complaint Inspection | No Violations Noted | Complaint insepction of conventional well in production. Well is dilapidated, and appears to be abandoned. Contacting operator to discuss plugging plan. No violations noted at this time. |
2512495 | Primary Facility | 2016-08-19 | Routine/Complete Inspection | No Violations Noted | Inspection of the Gughiocello 1H gas well in Chapman Twp. Clinton Co. conducted August 19, 2016 at 1:00 pm. The well has “active status” however the listed operator “New York Petro Min Corp” appears to be gone and the well is abandoned. The well is up on the hill behind Grant Ave. in Chapman Twp. At the time of inspection; The well is shut-in with pressure on it. I measure the outer casing to be 7” with 2 7/8” tubing installed, consistent with old well record found at the office. The well is TDd at 2800', Lock Haven formation. From the tubing string the well is plumbed to production equipment; an old glycol treater to pipeline. The well is open to taylor-plug valves on the well head which will be useful to check pressure on the well if or when it gets plugged. I checked the well for gas using an Altair 5x gas meter and found methane outside the tubing, inside the 7”. |
2546911 | Primary Facility | 2016-12-22 | Routine/Complete Inspection | No Violations Noted | Inspection of the Gughiocello 1 conventional gas well in Chapman Twp. Clinton Co. conducted December 22, 2016 at 10:00 am as part of an abandoned well investigation. The well is on a forested hill behind Grant Ave (North Bend) approximately 700’ from a nearby water supply complaint and the nearest “active” well to the residence. Since my last inspection 8/19/16 a search was conducted on the last known operator; “NEW YORK PETRO MIN CORP” yielded no results. It is likely this was an old existing well that this company reentered and reworked because the only well record on file is from 1979 and the information for the 7” (cement/date run) was unknown but shows in 1978 they ran 2 7/8” tbg to 2600’, cemented w/150 sacks, perf’d from 2540’-2520’ and stimulated in 1979. This is the most recent activity I found on record, and no recent production is on record. At the time of Inspection; The well had a tubing head on the 2 7/8”. The well was shut-in, at the lines going out to production. The well had pressure on it a |
2691449 | Primary Facility | 2018-02-01 | Routine/Complete Inspection | No Violations Noted | On February 1st the Department was on location for a pre-bid meeting for an upcoming plugging contract. There were no obvious changes in the well's condition observed. |
2728566 | Primary Facility | 2018-05-10 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | A routine inspection of this orphan well was conducted May 9th, 2018. The well is being plugged under a state contract. At the time of inspection, the contractor has begun to clear a path to the well and moving in equipment. There were no significant changes observed to the well itself and no obvious issues were observed. |
2731040 | Primary Facility | 2018-05-16 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | A routine (plugging) inspection of the Gughiocello 1 conventional well in Chapman Twp., Clinton county (North Bend) was conducted May 16, 2018 at approximately 10:00 am. The well is being plugged under a state contract. The rig is on location for P&A. I met with the crew to discuss operations on the pad. Both the 2 7/8" and 1" are open and venting. There was reportedly ~900 psi on the well before blowing it down. They blew down the well, shut it in over night and pressure built up to ~600 psi the next day. The hole has not been entered yet. The crew intends to pull the 1" and set a bridge plug in the 2 7/8" to isolate the producing formation before cementing. No obvious major issues were observed. |
2731151 | Primary Facility | 2018-05-17 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | A routine, plugging inspection of this conventional well was conducted May 17, 2018. The well is being plugged under a state contract. Plants & Goodwin are rigged up on the well for P&A. At the time of inspection, all the 1" is out of the hole. Wireline is on location where they set a bridge plug in the 2 7/8" at 2400' to isolate high pressure from the productive interval prior to cementing. Wireline is logging the cased hole at this time to determine the cement top behind the 2 7/8" for pipe retrieval. Fluid inside the casing is too foamy to get a good signal on the log at this time. The plugging contractor is loading the hole from the top of the casing with water too address the issue. Some venting was observed from the 2 7/8" which may be residual gas that was dissolved in fluid and/or or trapped above bridge plug making it's way out of the wellbore. The level of venting is reportedly less now than prior to setting the bridge plug. No obvious major issues were observed. |
2735180 | Primary Facility | 2018-05-24 | Routine/Complete Inspection | No Violations Noted | A routine (plugging) inspection of the Gughiocello 1 conventional gas well was conducted May 24, 2018. The well is in the process of being plugged under a state contract. I arrived on location at 1:00 pm. Plants and Goodwin are rigged up, along with wireline on location for logging and a bridge plug. At the time of inspection; the well was venting from the 2 7/8". The well has continued to make gas after setting previous bridge plug and pressure reportedly builds to 150-200 PSI overnight. I reviewed a temperature log with the wireline operator. The log indicated gas may be coming in the 2 7/8" where it is likely compromised around ~1500'. The contractor ran in hole with a a mechanical bridge plug and set it at 1460' to isolate the issue. The venting appeared to be diminishing as the rig was loading the hole with water from top in preparation for wireline to run a CBL to determine TOC behind the 2 7/8". I left location at 3:15 pm. |
2735760 | Primary Facility | 2018-05-29 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | A routine (plugging) inspection of the Gughiocello conventional well was conducted May 29, 2018. The well is in the process of being plugged under a state contract. At the time of inspection, Plants and Goodwin are rigged up on the well along with a pump truck on location to mix and pump final cement plug. I spoke with the contractor to discuss operations on the pad. After setting the mechanical bridge plug (1460') last week, wireline ran back in with a CBL, determined TOC behind the 2 7/8" to be shallow (~30'-60'). 2 7/8" will stay in the hole. They shut the well in over the long weekend and opened up to a gauge this morning. There was reportedly no pressure this morning. At the time of inspection, they are on top of the bridge plug with 1" workstring in the hole, cement truck rigged up to pump. They broke circulation immediately and spotted plug from bottom back to 1030'. Slurry looked ok, displaced with water. Good returns were observed while pumping. Crew broke off and is preparing to pull workstring. No |
2737839 | Primary Facility | 2018-06-04 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | A routine inspection of this plugged conventional well was conducted June 4, 2018. Plants and Goodwin have finished plugging this well under a state contract. At the time of inspection, a 2" vent pipe has been installed inside 2 7/8" (unrecoverable based on CBL results, TOC @ ~30-60') with marker plate and will be grouted later today as a plugged well monument. There was no audible or visible flow inside or outside the vent, and no gas was detected with an Altair 5x gas meter. The contractor is onsite beginning reclamation. No obvious issues were observed. |
2740287 | Primary Facility | 2018-06-12 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | A routine inspection of this plugged conventional well was conducted June 12th, 2018. The well has been plugged under state contract OOGM 17-2. This inspection will serve as the final inspection for the work completed under the contract. At the time of inspection, all equipment has been demobilized from the access road and the well location, and all necessary work to restore the access road and location to original or better condition has been completed. A 2" vent with marker plate has been grouted in the top joint of 2 7/8" extending approximately 6' above ground level. There was no audible or visible flow coming from the vent, verified by a field reading with an Altair 5x gas meter which did not detect gas in any amount at the vent. No gas was detected outside the 2 7/8" at ground level. No obvious issues were observed during this inspection. At the time of inspection it appears that the work has been satisfactorily completed in accordance with the plans, and specifications outlined in contract number OOG |
2770599 | Primary Facility | 2018-08-31 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | A routine inspection of this plugged, orphan well was conducted August 31, 2018. The well was plugged under a state contract in May of this year. At the time of inspection; the well has a 2" vent pipe with marker plate grouted in 2 7/8" tubing as a plugged well monument. There were no open holes or discharges observed. There was no audible or visible venting observed. No gas was detected at the mouth of the vent pipe with an Altair 5x gas meter. No gas was detected outside of tubing at ground level. No obvious issues were observed. |
