1973352 Primary Facility 2011-05-16 Complaint Inspection No Violations Noted Todays inspection was in response to a complaint called into the office alledging debris was coming off this gas well site, and it had caused the road culvert to plug and flood the Cooper vineyard. Upon inspection I meet with the complaintant and looked at the site. This site has not had any earth moving. My observation was that the site did not cause the culvert to plug. This was based on the fact that the site is small, has a very slight slope, and was grass covered. My observation was the water flow from the south, in the road ditch carry the debris and plugged the pipe. No violations.
1976664 Primary Facility 2011-05-16 Routine/Complete Inspection No Violations Noted Today's inspection was the result of a inquiry re: the possibility of site run-off plugging a road culvert and causing flooding on the west side of Dewey Road. Upon inspection I meet with the neighbor alledging the problem was caused by site run-off and we looked at the site and road culvert together. Upon inspection I found the site is grass covered; no earth moving has taken place.The only site work done so far is vine removal. I observed a very slight evidence of small amount of runoff coming from the site area. I saw no evidence of debris leaving the site. I explained to the landowner that I felt the material that had plugged the road culvert had orginated in the large drainage area feeding the large road ditch. No violations
2004659 Primary Facility 2011-09-22 Site Restoration No Violations Noted this gas well was never drilled; however the site was partially constructed no earth moving had taken place only site clearing. Upon inspection I found a 99% cover over the well pad site.
2007130 Primary Facility 2011-09-28 Site Restoration No Violations Noted This site was cleared but no dirt was moved. and the drill rig was never brought in. Upon inspection I found the site had 98 % ground cover established. Restoration is complete.
