1953724 Primary Facility 2011-02-28 Routine/Complete Inspection No Violations Noted The purpose of my inspection today was to determine if the E&S controls that have been installed on this new Well Pad Site are preventing sediment from leaving the site during this heavy snow melt and rain. Upon inspection I found all slit contained on the site and the slit fence was intact, and not overtopping. The Access Road was dozed in, and rock base applied. The site was rough graded; the pits have not been excavated. The E&S Plan was on-site and implemented as required. Slit fence surrounded the earth disturbance area. The Well Site ID Tag was on site. No violations noted
1973301 Primary Facility 2011-05-10 Routine/Complete Inspection No Violations Noted Upon inspection I found the E&S plan and permit on-site. I also found the slit fence in good repair. The site needs reworked to direct water flow around the north side of the top soil pile to get it off the well pad site. Access road stoned and and well pad approx 65% stone covered . No violations
