Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1624278 Primary Facility 2007-05-17 Routine/Complete Inspection No Violations Noted FILTER FENCE NEEDS MTCE. NO SEE/MULCH YET. ROUGH GRADED.
2131600 Primary Facility 2013-01-16 Routine/Complete Inspection No Violations Noted Site Id sign in place, tank in containment, well labeled, equipment fenced off, no leaks observed today.
2360526 Primary Facility 2015-04-02 Complaint Inspection No Violations Noted Marcellus. Site ID sign present, in compliance in Act 9 standards. ERP present at access road entrance.
2572539 Primary Facility 2017-03-17 Routine/Complete Inspection No Violations Noted The Act 9 sign was in place and all required documents were present at the site. The site had been recently plugged and appeared to have been recently restored. There were no signs of erosion observed.
2575143 Primary Facility 2017-03-24 Plugging(Includes Plugged/Mined Through) No Violations Noted During Final Inspection by WQS early this week indicates issues with expose 2" pipes on site. One could have low pressure of gas. The discussion is who is required to address these concerns. The operator on record believes these concerns are past operator concerns.
2575934 Primary Facility 2017-03-24 Routine/Partial Inspection No Violations Noted The site was lightly mulched, but did not appear to be seeded. It is recommended that the site be seeded and receive additional mulching to prevent erosion. An erosion channel had formed upslope of the access road and pad and sediment was observed near the cross drain. An erosion rill in the access road was observed. It is recommended these be repaired to prevent further erosion. Silt socks were present at the site. It is recommended that these be removed and any soil exposed from the removal be seeded and mulched. A pipe was found in the grass between the access road and well pad on the downslope side. It is recommended the pipe be removed.

The well marker was in place and clearly visible. What appeared to be production lines and equipment were observed on the pad (see photographs). It is requested that the lines and equipment be identified and a document depicting the location of underground infrastructure be provided to the Department.
