On 12/21/16 at approximately 1100 hours I conducted a routine complete inspection of the Whyel unit. The Act 9 sign was in place and all required documents were present. A gravel pad and road to the pad was constructed in the location of the Department approved flow back impoundment (see photographs). The location was to be restored to approximate original grade and was to be vegetated. This is a violation of 102.5(m)(4). |
