2780113 Primary Facility 2018-09-20 Complaint Inspection Violation(s) Noted On 09/20/18 the Department received a complaint alleging a slip had occurred in the early part of 2018 on the access road to the Yost #14 well.

On 09/20/18 at approximately 1200 hours I conducted an inspection of the site. The slip appeared to be approximately 250' to 300' long. Additional cracks were observed in the access road. The well can not be safely accessed.

Environmental Group Manager Dave McDermott contacted Kirk Elkin of Diversified and requested DGO staff assess the site the following week and provide a plan to remediate the slip.
