The Act 9 sign was clearly legible at the site entrance. The mailbox containing required documentation associated with the site had fallen off of the post it was attached and was lying on the ground. All required documents were available on-site although the documents were water-logged and becoming illegible. The operator should repair the mailbox and confirm that all required documents are present and in good legible condition. The access road and the well pad were in good condition. Each could use some minor maintenance, especially following the recent precipitation. The site has been stabilized with vegetation. The pad and the surrounding area were observed to be used to graze cattle. The Act 9 sign was clearly legible at the site entrance. The access road which has two spurs to access the GH5SW CBM Well (located above GH34 Pad) and the GH5 CBM Well (located prior to the GH34 Pad) was generally in good condition. An area of erosion was observed at the beginning of the spur to access the GH5 CBM Well and will be addressed in a separate inspection report for the GH5 Site. The site has been stabilized with vegetation. The surrounding area (including the site) is currently used to graze cattle. The earthen berm secondary containment around the production tank appeared to be in good condition and was observed to be vegetated. The production tank also appeared to be in good condition. Inspection of the area of concern confirmed that the site mailbox had been mounted to a post and all site documentation was replaced with new legible copies, which included an Emergency Response Plan. The concerns identified in SAIR No. 2840023 have been addressed. No violations were identified during the inspection. |
